摘  要:目的探讨血清γ-谷氨酰转移酶(GGT)水平与急性脑梗死(ACI)CISS病因分型中大动脉粥样硬化性及穿支动脉病变的相关性。方法急性脑梗死住院患者221例,依据临床症状、体征、头磁共振、头颈部CTA、颈动脉超声、DSA检查结果分为大动脉粥样硬化性组(n=119)、穿支动脉病变组(n=102)。所有入选病例均记录吸烟、饮酒、高血压、糖尿病及冠心病等动脉粥样硬化的危险因素,采用酶法检测GGT、血脂、血糖等相关生化指标。所有数据均采用SPSS17.0统计软件进行统计分析。结果大动脉粥样硬化性组GGT水平显著高于穿支动脉病变组。在除外其他动脉粥样硬化相关危险因素后,大动脉粥样硬化组GGT水平仍显著高于穿支动脉病变组,差异有统计学意义。大动脉粥样硬化组及穿支动脉病变组患者在卒中病史上的差异无统计学意义。结论 GGT水平对于急性脑梗死CISS分型中大动脉粥样硬化性及穿支动脉病变有提示作用,在动脉粥样硬化的发生发展中起重要作用。两组患者在急性脑梗死病史上差异无统计学意义。Objective To investigate the correlation of serum gamma-glutamyl transferase levels with large artery atherosclerosis and penetrating at-tery disease in the CISS typing in acute cerebral infaretion. Methods A total of 221 patients with acute cerebral infarction were recruited for the study. On the basis of clinical symptoms, signs, head magnetic resonance, head and neck CTA, carotid artery ultrasound, DSA examination results,the patients were divided into large artery atherosclerosis group (n = 119)and penetrating artery disease group (n = 102). The risk factors of athero- sclerosis were recorded in all these patients, including smoking and di-inking history, hypertension, diabetes mellitus and coronary heart disease. Thelevels of GGT, blood fat and blood glucose were measured. SPSS17.0 statistical software was used for statistical analysis. Results GGT levels in pa- tients of large artery atherosclerosis group were significantly higher than penetrating artery disease group. In except of other risk factors of atherosclero- sis, GGT levels in patients of large artery atherosclerosis group were also significantly higher than penetrating artery disease group. There was no sig- nificant difference in the medical history of acute cerebral infarction between large artery atherosclerosis group and penetrating artery disease group. Conclusion GGT level has a prompt action for the CISS typing of large artery atherosclerosis and penetrating artery disease in acute cerebral infarc- tion, and also plays an important role in the occurrence and development of atherosclerosis. The two groups have no difference in the medical history of acute cerebral infarction.
