16排螺旋CT诊断自发性结肠破裂的应用价值

摘　　要：目的探讨16排螺旋CT在自发性结肠破裂中的诊断价值。方法回顾性分析经手术病理证实的12例自发性结肠破裂的CT影像资料及临床资料,采用16排螺旋CT扫描,3例急诊CT平扫,9例均行平扫加增强扫描,获得图像采用多种后处理技术。结果 12例自发性结肠破裂CT表现为腹腔游离气体10例、肠系膜小气泡征5例、肠外粪块8例、破裂口显示3例、毗邻破裂口肠壁增厚水肿3例、弥漫性腹膜炎4例、破裂口周围局限性渗出9例、腹腔或腹膜后区积液5例。结论 16排螺旋CT在诊断自发性结肠破裂上具有重要价值,为临床手术治疗提供有利依据。Objective To evaluate the value of 16‐slice Spiral CT in diagnosis of spontaneous rupture of the colon .Meth‐ods The clinical data of 12 cases with sptontaneous colonic perforation confirmed by surgery and pathology were analyzed retrospectively .Plain CT scans were performed in 3 emergency cases ,contrast‐enhanced CT scans were performed in 9 pa‐tients .VR ,MIP ,MPR ,CPR ,MinP of the source images were followed .Results The spiral CT signs included perito‐neal cavity free gas in 10 cases ,mesenterium small bulles in 5 cases ,feces out of the colon wall in 8 cases ,rupture cut dis‐played in 3 cases of the wall of colon adjacent rupture holes thickened and edematous ,and diffused the peritonitis in 4 ca‐ses .Transudation around the colon perforation in was found 9 cases and peritoneal or retroperitoneal hydrops in 5 cases . Conclusion 16‐slice spiral CT plays an important role in the diagnosis of spontaneous colonic perforation ,provides favour‐able basis for clinical surgery .
