小儿脂肪母细胞瘤CT及MR影像分析

摘　　要：目的探讨小儿脂肪母细胞瘤的CT和MRI表现,提高对本病的认识。方法收集经我院手术及病理证实的11例小儿脂肪母细胞瘤,结合相关文献分析其CT和MRI表现。结果位于颌下者1例,胸壁3例,纵膈1例,腹腔内3例(均附着于肠系膜),下肢3例。其中局限型7例,弥漫型4例。CT及MRI上均表现为含脂肪组织密度或信号的肿块,病灶呈分叶状(8/11例),肿块内密度信号均不均匀,其内可见多少不等的软组织成分及纤维分隔,其中3例存在显著多发纤维分隔而使病灶内部呈现多分叶样改变;2例病灶内可见明显囊变。6例肿瘤边缘见光滑包膜,5例肿瘤与周围组织结构分界不清。6例增强扫描病变均呈不均匀强化,表现为软组织成分及分隔的轻度强化。结论根据脂肪母细胞瘤影像表现特征,并结合患儿年龄及临床表现有利于本病的正确诊断。Objective To explore the imaging features of lipoblastoma in children .Methods Clinical data and imaging findings of 11 cases of children with lipoblastoma were retrospectively analyzed .Results Lesions were located in submax‐illary space 1 case ,chest wall in 3 cases ,mediastinum in 1 case ,abdomen in 3 cases ,lower extremities in 3 cases .7 cases were localized ,and the other 4 cases were diffused .The imaging findings on CT and MRI demonstrated a lobulated lesion with fatty density or signal intensity ,including the presence of fibrous septa ,cystic changes ,and soft tissue components . 6 patients showed mild heterogeneous enhancement of soft tissue components and septa .Conclusion The imaging features of lipoblastoma are similar to some fat‐containing neoplasms and unspecific .We can make a correct diagnosis combining with the age of children and clinical features .
