经内镜套扎治疗消化道黏膜下肿瘤的临床研究

摘　　要：目的探讨经内镜套扎术对消化道黏膜下肿瘤的治疗价值。方法常规电子内镜下明确的消化道黏膜下隆起病变,在行电子超声内镜检查确诊为黏膜下肿瘤后,采用内镜下套扎术治疗。结果确诊黏膜下肿瘤43例,其中间质瘤22例,平滑肌瘤18例,脂肪瘤3例;来源于黏膜下层11例,来源于固有肌层32例。采用胶圈套扎治疗25例,尼龙绳套扎治疗18例。肿瘤脱落42例,成功率98%。术后发生食管狭窄1例,并发症发生率2.3%。结论经内镜套扎治疗消化道黏膜下肿瘤是一种安全、有效的治疗手段,值得在无经内镜黏膜下剥离术(ESD)手术条件的基层医疗单位推广应用。Objective To evaluate the clinical application of endoscopic ligation technique on gastrointestinal submucosal tumor. Methods 43 cases of submucosal tumor were diagnosed by endoscopic ultrosonography and treated with endoscopic ligation technique.Results With aprons ligation treatment of 25 cases, With nylon rope ligation treatment of 18 cases, The successful rate of tumor removal was 98%. There was a post-ligation esophagus stricture case. Conclusions Endoscopic ligation technique treated submucosal tumor is a simple,safe and effective way. It is necessary to popularize this method in the lower grade hospital.
