基于医案多元统计分析的明清时期黄疸病用药规律研究

摘　　要：目的:研究明清医家治疗黄疸病用药规律。方法:搜集《中华医典》中明清时期的黄疸病医案,对医案的中药组成、功效归类、性味归经等参照相关标准进行数据规范化处理,并采用频数分析、聚类分析等方法进行统计分析。结果:频数分析显示明清时期黄疸病用药以利水渗湿药、补益药、清热药为主,辅以理气、化湿、解表等药,药味以苦、甘为主,药性则以寒性和温性为主;药物多归脾胃经。通过聚类分析归纳出茵陈五苓散和栀子柏皮汤为主的核心药物群,具体药物包括:茵陈、茯苓、猪苓、白术、厚朴、陈皮、栀子、甘草等。结论:明清时期黄疸病的用药方面具有祛湿为先、重视脾胃、寒温并用、攻补兼施等鲜明的特点,重治法而不重成方、用药均衡平和。Objective： To study the medication laws hided in Chinese medicine case reports in Ming and Qing Dynas-ties. Methods ： We collected the jaundice medical records of Ming and Qing Dynasties and arranged medicines and createdthe database and analyzed with statistics method including Frequency analysis and Cluster analysis. Result： 51 books and230 cases have been collected. With frequency analysis, dampness - draining medicines, heat - clearing medicines andtonifying medicines were the main medications, followed by qi - regulating medicines, dampness - dispelling medicinesand exterior - releasing medicines. It showed the main therapeutic principles were resolving dampness, reinforcing spleen- stomach and clearing heat. Regulating qi and dispelling dampness and releasing the exterior were other commonly -used principles. （1）The most common tastes of high - frequency medicines were bitter and sweet tastes. The natures ofthem were cold and warm. The related meridians concluded spleen meridian and stomach meridian. （2）With cluster analy-sis, medicines in the core group were as following： capillaris, Poria, Polyporus, Atractylodes, Magnolia gardenia, orange peeland licorice. It based on Yinchen Wuling powder and Zhizi Bopi Decotion. Conclusion： In medication, the features con-cluded dispelling dampness, emphasizing spleen - stomach, treating with both cold and heat, treating with both eliminationand reinforcement and emphasizing not specific medication but therapeutic principles and balance.
