直肠外翻技术在腹腔镜直肠前切除术中的应用研究

摘　　要：目的探讨直肠外翻技术在腹腔镜直肠前切除术中应用的安全性、可行性。方法回顾分析联合使用直肠外翻技术和腹腔镜直肠前切除术治疗的12例直肠癌患者手术情况、肿瘤根治效果、并发症及短期疗效。结果12例患者均顺利完成直肠外翻和腹腔镜直肠前切除术，无中转开腹，均未发生严重的术中并发症及手术死亡。手术平均时间为200（150～260）min，术中平均出血48（20～100）ml，术后平均排气时间、留置尿管时间及术后住院天数分别为3．0（2．4～4．2）天、5．1（3—7）天和9．3（7—14）天，患者平均清扫淋巴结19（14～28）个，肿瘤下缘距切缘平均距离为3（2—4）em。所有患者均获随访，中位随访时间18．5（7～30）个月，未发现局部复发和转移，术后患者排便功能恢复良好。结论直肠外翻技术联合腹腔镜直肠前切除术安全、可行，美容效果好。Objective To investigate outcomes of the evening technique in patients who underwent laparoscopy - assisted anterior resection of rectal cancer. Methods The surgical technique, oncological outcome, complications, and the short - term results were studied in 12 patients who undergoing laparoscopy - assisted anterior resection with a everting technique for rectal cancer. Results Smooth operation was completed in all the 12 patients who undergoing laparoseopy - assisted anterior resection with a everting technique for rectal cancer. No severe intraoperative complications and death occurred in all patients. No patients were converted to open operation. The mean operation time was 200 （150 -260） min and the mean quantity of blood loss was 48 （20 - 100） ml. The mean time of postoperative exhaust time, indwelling catheter time, and the length of postoperative hospital stay were 3.0 （2.4 - 4.2） days, 5 （3 - 7） days, and 9.3 （7 - 14） days, respectively. The total number of lymph nodes harvest was 19 （14 -28） per case, and the distal margin from the tumor was 3 （2 -4） cm. The median follow -up time was 18.5 （7 -30） months. No local recurrence or metastasis was observed. Acceptable anal function results were obtained in the patients. Conclusions Laparoseopy - assisted anterior resection with the everting technique is a feasible and safe radical procedureand it also has good cosmetic effect.
