冬季与夏季椎-基底动脉系统短暂性脑缺血发作临床特点比较

摘　　要：目的探讨冬、夏季节椎-基底动脉系统短暂性脑缺血发作(VBAS-TIA)患者临床特点的差别。方法225例VBAS-TIA患者按照发病时间分为冬季组(134例)和夏季组(91例),比较组间的性别、年龄和伴发高血压、糖尿病、高脂血症、心脏病的例数;比较组间的血压、左心室射血分数、心输出量、颈部血管血流速度以及颈动脉粥样硬化(AS)、重度狭窄、椎动脉迂曲/纤细的例数。结果与冬季组比较,夏季组患者的年龄较轻、男性和伴发高血压、高脂血症的例数均较少(P<0.05);伴发糖尿病、心脏病的例数均差异无统计学意义(P>0.05)。与冬季组比较,夏季组患者的血压、颈部血管血流速度以及AS、重度狭窄、椎动脉迂曲/纤细的例数均较少(P<0.01);左心室射血分数、心输出量的比较均差异无统计学意义(P>0.05)。结论季节因素可能影响着不同时期VBAS-TIA的临床特点。Objective To explore the difference of clinical characteristics of vertebral basilar artery system transient ischemic attack（VBAS-TIA） in winter and summer. Methods According to the time of onset, 225 patients with VBAS-TIA were divided into winter group （ n = 134） and summer group （ n = 91）. Then, gender, age and the numbers of patients complicated with hypertension, hyperlipidemia, diabetes and heart disease in the two groups were compared. Meanwhile, blood pressure, left ventricular ejection fraction, cardiac output, neck vascular blood flow velocity and the numbers of patients suffering from atherosclerosis（AS）, stenosis and vertebral artery tortuosity/slim were also compared. Results Patients in the summer group were younger than those in the winter group and there were less male patients in the summer group （ P 〈0.05）. The number of patients complicated with hypertension or hyperlipidemia was also markedly less in the summer group than in the winter group （ P 〈0.05）. However, there were no statistically significant difference in the cases complicated with diabetes and heart disease between the two groups （ P 〉0.05）. Compared with the winter group, the blood pressure and neck vascular blood flow velocity in the summer group were prominently lower and the cases suffering from AS, stenosis and vertebral artery tortuosity/slim were significantly less than those in the winter group （ P 〈0.01）, but the left ventricular ejection fraction and cardiac output showed no significant difference between the two groups （ P 〉 0.05）. Conclusion Seasonal factors may influence the clinical features of VBAS-TIA in different phases.
