枸橼酸坦度螺酮治疗脑卒中后抑郁的临床效果和安全性

摘　　要：目的 观察枸橼酸坦度螺酮治疗脑卒中后抑郁的临床效果和不良反应.方法 选取来我院诊治的78例脑卒中后抑郁患者,随机分为治疗组40例和对照组38例.对照组给予常规治疗和心理治疗;治疗组在对照组治疗基础上给予枸橼酸坦度螺酮治疗,每次10 mg,3次/d口服.两组疗程均为4周.治疗前后进行汉密尔顿抑郁量表（HAMD）评分,并对其临床疗效和药物的不良反应进行观察比较.结果 （1）治疗组HAMD评分在治疗2周为（8.09±2.21）分,4周为（6.31±2.28）分,与治疗前[（11.98±2.17）分]比较明显下降,差异均有统计学意义（P均＜0.05）;治疗2、4周与对照组[（10.57±2.53）分和（8.91±2.31）分]比较差异均有统计学意义（P均＜0.05）.（2）对照组治疗总有效率为31.6％（12/38）,治疗组为92.5％ （37/40）,两组比较差异有统计学意义（x^2=63.69,P＜0.01）.（3）治疗过程中两组均未出现明显不良反应.结论 枸橼酸坦度螺酮治疗脑卒中后抑郁效果明显,未见明显不良反应.Objective To investigate the clinical efficacy and side reaction of tandospimne citrate capsules on treating patients with depression after stroke.Methods Seventy-eight patients who were diagnosed as anxiety and depression after stroke were taken as our subjects.They were randomly divided into control group （n =40） and treatment group （n =38）.Patients in the control group were given conventional therapy for cerebral vascular disease and psychological treatment,while in the treatment group were given the same treatment besides tandospimne citrate capsules of 10 mg per times,3 times/d oral,and course of both two groups for 4 weeks.The patients of two groups were scored according to Hamilton Depression Scale（HAMD） at the time of pre-treatment and post-treatment.And the the clinical efficacy and side reaction were observed and compared.Results （1） HAMD scores of treatment group in 2,4 weeks after treatment had significantly lower than that before treatment （（8.09±2.21） vs.（6.31 ±2.28） vs.（11.98 ±2.17）,P 〈0.05）.HAMD scores of control group in the treatment at 2week was as same as that in 4 weeks（（10.57 ±2.53） vs.（8.91 ±2.31））.There was statistically significant difference in two groups in 2,4 weeks after treatment in terms of HAMD scores（P 〉0.05）.The total effective rate of control group was 31.6% （12/38）,and 92.5% （37/40） in treatment group,and the difference was significant（x^2 =63.69,P 〈 0.01）.In the course of treatment in the two groups,there were no obvious adverse reactions.Conclusion Tandospirone citrate capsules show the clinical curative effect in the treatment of anxiety and depression after stroke,and no obvious adverse reactions.
