摘  要:目的:探讨应用肾动态显像对肺癌术后大剂量顺铂(DDP)化疗方案患者的早期肾功能损害评价的临床价值。方法:61例肺癌术后首次应用大剂量顺铂为主化疗方案的患者,分别在术前和术后化疗1个周期结束时应用肾动态扫描测量肾小球滤过率(GFR)和实验室化验血清肾功尿素氮(BUN)、肌酐(Cr)以及尿蛋白β2-微球蛋白(β2-MG)、α1-微球蛋白(α1-MG)指标。结果:61例患者化疗后GFR下降16%(t=8.46,P<0.05),肾功(BUN、Cr)、尿蛋白(β2-MG、α1-MG)变化不明显(P>0.05)。结论:肾动态显像监测GFR能够评价肺癌患者化疗后早期肾脏功能损害,具有早期诊断价值。Objective: To explore clinical value of evaluating the early effect of chemotherapy on renal function using renal dynamic imaging and laboratory tests of renal function and urinary protein in patients with lung cancer treated with high-dose cisplatin(DDP). Methods: Among 61 cases of lung cancer after the first application of high-dose cisplatin chemotherapy, respectively in the preoperative and postoperative chemotherapy for 1 cycle, Glomerular filtration rate(GFR), blood urea nitrogen(BUN), creatinine(Cr),and urinary β2- microglobulin(β2-MG), microglobulin(α1-MG), were tested using renal dynamic scanning.Results: GFR in 61 patients after chemotherapy was obviously decreased 23% in comparison with that before chemotherapy(t=8.46, P〈0.05).However, it was not significant change in renal function(BUN, Cr), urinary β2- microglobulin(β2-MG) and microglobulin(α1-MG)(P〉0.05). Conclusion: renal dynamic imaging is a valuable method in early diagnosis by measureing GFR to early functional damage valuation of patients with lung cancer after chemotherapy. It is worthy of popularization and application for clinicians.
