后循环短暂性脑缺血发作和后循环脑干梗死患者脑干听觉诱发电位变化及其临床意义

摘　　要：目的：观察后循环短暂性脑缺血发作（ TIA）和后循环脑干梗死患者脑干听觉诱发电位（ BAEP）变化，探讨其临床意义。方法选择我院2011年7月—2013年12月收治的后循环TIA患者38例（后循环TIA组）和后循环脑干梗死患者46例（后循环脑干梗死组），均在发病后24 h内、24 h、1周进行BAEP检查，比较两组患者Ⅰ、Ⅲ、Ⅴ波潜伏期和Ⅰ～Ⅲ、Ⅲ～Ⅴ、Ⅰ～Ⅴ波峰间潜伏期。结果两组患者发病后24 h内、24 h、1周Ⅰ波潜伏期和Ⅰ～Ⅲ波峰间潜伏期比较，差异均无统计学意义（ P＞0.05）；后循环脑干梗死组患者发病后24 h内、24 h、1周Ⅲ、Ⅴ波潜伏期和Ⅲ～Ⅴ、Ⅰ～Ⅴ波峰间潜伏期均长于后循环TIA组（ P＜0.05）。结论后循环脑干梗死患者Ⅲ、Ⅴ波潜伏期和Ⅲ～Ⅴ、Ⅰ～Ⅴ波峰间潜伏期变化较后循环TIA患者明显， BAEP检查对后循环TIA和后循环脑干梗死具有一定的鉴别诊断价值。Objective To investigate the changes and clinical significance of brain stem auditory evoked potentials （ BAEP） of patients with posterior circulation transient ischemic attack （ TIA） and patients with posterior circulation brain stem infarction.Methods From July 2011 to December 2013 in our hospital, 38 patients with posterior circulation TIA were selected as A group , and 46 patients with posterior circulation brain stem infarction selected as B group.Both groups underwent BAEP ex-amination within 24 h, at 24 h and at 1 week of onset, peak latency （PL） of Ⅰ, Ⅲ, Ⅴ waves and inter -peak latency （ IPL） of Ⅰ-Ⅲ, Ⅲ-Ⅴ, Ⅰ-Ⅴwaves between the two groups were compared .Results There was no significant differ-ences of PL ofⅠor IPL ofⅠ-Ⅲwave between the two groups within 24 h, at 24 h and at 1 week of onset （P〉0.05）; while PL ofⅢ, Ⅴwaves and IPL of Ⅲ-Ⅴ, Ⅰ-Ⅴwaves of B group were longer than those of A group within 24 h, at 24 h and at 1 week of onset （P〈0.05）.Conclusion PL of Ⅲ, Ⅴ waves and IPL of Ⅲ-Ⅴ, Ⅰ-Ⅴwaves of patients with posterior circulation brain stem infarction are significantly longer than those of patients with posterior circulation TIA , and BAEP examina-tion has a certain value in differentiation diagnosis of posterior circulation TIA and posterior circulation brain stem infarction .
