老年结直肠癌患者病理分期与肿瘤相关性贫血的研究

摘　　要：目的研究老年结直肠癌患者肿瘤相关性贫血发生情况,探讨肿瘤病理分期与贫血间的关系,为肿瘤相关性贫血的干预提供依据。方法选择湖北医药学院附属太和医院2013年4月—2014年4月收治的老年结肠癌患者245例作为研究对象,收集患者Hb水平,采用Dukes分期法收集患者的肿瘤分期情况。贫血诊断标准采用美国国立肿瘤研究所贫血分级标准,贫血细胞形态学分类参照《内科学》第8版教材。结果 245名老年结直肠癌患者Hb平均值为(108.16±22.02)g/L,贫血发生率为54.3%(133/245),不同性别患者肿瘤相关性贫血发生率差异无统计学意义(P>0.05),不同病理分期患者贫血发生率差异有统计学意义(P<0.05);不同病理分期患者贫血类型差异有统计学意义(P<0.05);不同肿瘤部位患者贫血发生率差异有统计学意义(P<0.05)。结论老年结直肠癌患者贫血发生率较高,贫血的发生与肿瘤病理分期密切相关,抗肿瘤治疗的同时应积极干预患者贫血状态,提高患者的机体抵抗力与生活质量。[ Objective] To study the incidence of anemia occurred in elderly patients with colorectal cancer tumor, to explore the re- lationship between tumor pathological staging and anemia, and provide basis for intervention of cancer-related anemia. [ Methods ] A total of 245 elderly patients with colorectal cancer treated in Taihe Hospital of Hubei Medical College from April 2013-April 2014were recruited as the research objects. The Hb level of patients was measured. Dukes staging method was used to identify the patient~ tumor staging. The anemia classification standard of the national cancer institute was applied to diagnose anemia. The eighth edition of the internal medicine textbook was taken as reference to conduct the anemia cell morphological classification. [Results]The average HB level of 245 elderly patients was （108. 16 ± 22.02） g/L. The incidence of anemia was 54. 3% （133/245 ）. The incidence of cancer-related anemia was not significantly difference between genders （ P 〉 0.05 ）. The incidence of anemia was significantly different between patients with different pathological stages （ P 〈 0.05 ）. The anemia types in patients with different pathological stages had significant differences （ P 〈 0.05 ）. The incidence of anemia in patients with different tumor sites had significant differences （P 〈0.05）. [ Conclusion] The incidence of anemia is high in elderly patients with eoloreetal cancer. There was a close relationship between the occurrence of anemia and tumor pathologic stage. It is necessary to take active measures against anemia and tumor simhaneouslv, so as to improve the body resistance and life quality of the patients.
