伴有黄疸症候的HBV相关性原发性肝癌患者外周血粒淋比及自然杀伤细胞计数的特征分析

摘　　要：目的:探讨伴有黄疸症候的HBV相关性原发性肝癌患者外周血中性粒细胞计数与淋巴细胞计数的比值(NLR)及NK(自然杀伤细胞)计数的特征。方法:收集2013年7月至2014年2月就诊于首都医科大学附属北京地坛医院中西医结合科及普外科的HBV相关性原发性肝癌住院患者,收集入组患者的一般临床资料,留取外周血,采用流式细胞术,分析T细胞亚群、NK细胞、B细胞的表达情况。结果:研究发现与无黄疸者相比,伴有黄疸症候出现的原发性肝癌患者NLR升高(P=0.028),NK细胞计数下降(P=0.029)。结论:与无黄疸症候的肝癌组相比,伴有黄疸症候的肝癌患者NLR升高,肝细胞损伤严重,肝脏合成和排泌功能下降,黄疸的出现和肝功能衰竭互为因果,因此NLR可以作为预测黄疸进展和肝功能衰竭的指标。Objective： To analyse the characteristic of neutrophil to lymphocyte ratio （NLR） and the absolute counting of circulating NK cells in patients with hepatitis B virus associated primary liver cancer （ HBV-PLC ） with jaundice. Methods： According to the inclusion criterria, we enrolled 60 HBV-PLC patients from July 2013 to February 2014 in Beijing Ditan Hospital, Capital Medical University. Based on flow cytometry, we analysed the frequencies of circulating T lymphocyte subsets, NK cells and B ceils. Results： Compared with HBV-PLC groups of no jaundice, we observed NLR was elevated in HBV-PLC groups with jaundice （P = 0. 028 ）, on the contrary, the absolute counting of circulating NK cells were declined （P = 0. 029 ） . Conclusion： Compared with HBV-PLC groups of no jaundice, we observed NLR was elevated in HBV-PLC groups with jaundice. The emergence of jaundice and liver failure reinforce each other. Therefore, NLR may be a marker to predict liver failure and guide of therapy.
