层级护理管理模式在骨科病房的应用效果实证分析

摘　　要：目的探讨层级护理管理模式在骨科病房的应用效果,为医院管理者提供理论学参考依据。方法从2012年3月开始在榆林市某三级乙等医院及交通大学某附属医院骨科病房开展层级护理管理模式,根据护士的能力设置不同层级,再根据层次的不同对每位护士进行相应的培训后在病房实施层次管理,用Epidate3.0和SPSS18.0软件比较在层次管理模式下及传统护理管理模式下的护士综合能力、护理工作质量、患者及家属的满意度及差错事故发生率的异同。结果采用层级护理管理模式后,临床护士的综合能力明显提高,临床护理能力、科研能力和教学能力均得到提高,差异有显著性(P均<0.05);护理工作质量评分明显改善,各个维度的得分比较均有显著性差异(P均<0.05);患者及其家属的满意度提高,差异有显著性(P均<0.05);差错事故发生率降低,差异有显著性(P均<0.05)。结论层级护理管理模式能够提高骨科病房护士的综合能力,增加患者家属及患者的满意度,减少差错事故的发生,提升骨科病房的伦理护理服务质量,层级护理管理模式可广泛应用在骨科病房的日常护理工作中。Objective： To explore the applied effects of nursing hierarchical management in orthopedics depart- ment, and then to offer some rationales and theoretical support for better hospital management. Methods：The fine hierarchical management was implemented from March 2012 in one top level hospital in Yulin and the affiliated hos- pital of Xi＇an Jiaotong university in Shaanxi Province. Before and after the hierarchical management carried out, the nursing quality was assessed by using satisfaction survey questionnaires; mistake accident rate and nurses＇ compre- hensive ability. The Epidate 3.0 and SPSS18.0 software was used to analyze. Results： After the nursing hierarchy management, the nurse comprehensive ability was improved, clinical nursing ability, scientific research ability and teaching ability were improved. The satisfaction scores of family members of hospitalized patients and patients were higher than that before （ P 〈 0.05 ）. Also, mistake accident rate was significantly reduced after the hierarchical man- agement（ P 〈0.05 ）. Conclusion ：The hierarchical management could improve the satisfaction of family numbers and patients, reduce the mistake accident rate, and enhance nursing care quality and improve the nurse compre- hensive ability. And further, it would reduce the incidence of medical malpractice. The fine management is worth to be applied and generalized in ethics nursing care of orthopedics nursing in orthopedics department.
