整体护理干预在急性白血病患者治疗期间的临床效果

摘　　要：目的：总结整体护理在急性白血病患者治疗期间的应用价值。方法回顾性分析2013年1月～2014年2月于海南省人民医院住院治疗的80例急性白血病患者的临床资料，将其分为两组，每组40例，对照组在常规治疗的基础上给予常规护理，观察组在对照组的基础上给予整体护理，比较两组患者在治疗期间心理状态的改善情况及临床治疗并发症的总发生率。结果对照组和观察组治疗前回避评分、屈服评分、社会支持度评分、汉密尔顿焦虑量表评分（HAMA）及汉密尔顿抑郁量表评分（HAMD）比较，差异无统计学意义（P〉0.05），经治疗后，对照组社会支持度评分有所改善（P〈0.05），但回避评分、屈服评分、HAMA及HAMD比较，差异无统计学意义（P〉0.05），观察组以上项目评分均显著改善，差异均有统计学意义（P〈0.05）。观察组回避评分、屈服评分、社会支持度评分、HAMA及HAMD评分治疗前后差值显著优于对照组（P〈0.05）。两组并发症发生情况比较，差异无统计学意义（P〉0.05）。结论整体护理可有效改善急性白血病患者的心理状态，并可降低治疗不良反应发生率。Objective To summarize the application value of holistic nursing during treatment of patients with acute leukemia. Methods The clinical data of 80 cases of acute leukemia from January 2013 to February 2014 in the Peo-ple＇s Hospital of Hainan Province were retrospective analyzed, and were randomly divided into two groups, 40 cases of patients in each group. The control group was given routine care on conventional therapy, the observation group was given holistic nursing care on the basis of the control group, the total incidence rate of improving mental state in the two groups during treatment and clinical treatment of complications were compared. Results The avoidance score, yield score, social support score, HAMA and HAMD of control group and observation group before treatment were not signif-icant different （P〉0.05）. After treatment, the social support score in the control group was improved, with significant difference （P〈0.05）, but the avoidance score, yield score, HAMA and HAMD were not significant different （P〉 0.05）, and the observation group were significantly improved in above project scores, with significant differences （all P 〈0.05）. The avoidance score, yield score, social support score, HAMA and HAMD before and after treatment in observa-tion group were significantly better than those in the control group （all P〈 0.05）. The occurrence of complications in two groups were compared, there was no significant difference （P〉0.05）. Conclusion The whole nursing can effective-ly improve the psychological status of patients with acute leukemia, and can reduce the incidence of adverse reactions in treatment.
