微创牙体预备方式对成人早期牙髓病治疗的临床应用

摘　　要：目的根据龋损的位置确定牙体的预备方式对成人早期牙髓病的微创治疗观察。方法严格掌握牙体预备方式的适应证：去净腐质后仅有颊侧和或某一髓角暴露,颌面剩余大部分健康牙体组织的患牙53例（例如：单纯颊面龋损和/或越过边缘嵴的临颌面龋）用微创方法进行牙体预备,刮净腐质及冠髓,保存根髓,2%戊二醛氧化锌糊剂一层覆盖根管口,丁香油水门汀、磷酸锌水门汀分层垫底,光固化复合树脂充填,术后随访2~3年,记录患者术后的全身症状、局部症状及对复诊次数治疗花费的满意程度,并对以上3方面做出评价。结果 153例均未发现全身症状;253例中49例未发现冷热刺激痛、夜间痛及自发痛,复诊时未发现牙折及叩痛,治疗成功率为92.5%,其余4例中有3例在2年内逐渐出现根尖炎症状,占总人数的5.7%,1例在术后5d出现慢性牙髓炎症状,占总人数的1.8%。3患者满意度调查结果为53例患者中50例患者对术中操作时间、复诊次数及治疗花费均感到满意,满意率为94.3%。结论微创牙体预备方式对适应证内的成人早期牙髓炎治疗效果令人满意。同时,与常规根管治疗牙体预备方法相比,减少对牙体组织的切割,最大限度地保存了健康牙体组织,达到了微创治疗的目的,有助于延长患牙的使用寿命。Objective To observe the effect of minimally invasive tooth preparation methods in the treatment of early adult dental pulp disease according to caries position.Methods Strict control of tooth preparation methods indications：go to the net humic only buccal and/or a medullary angle exposure,maxillofacial surplus major health of the teeth in53 cases（for example：simple buccal caries and beyond or marginal ridge Pro maxillofacial caries）were prepared with minimally invasive methods,scraped detritus and pulp,saved the root pulp,2% glutaraldehyde Zinc Oxide was used to past a layer to cover the root canal orifice,eugenol cement,zinc phosphate cement layered bottom,light cured composite resin filling.All patients were followed up for 2 ~ 3 years,the systemic symptoms,patients with postoperative local symptom and the satisfaction degree of spending on referral number of treatment were recorded,then the above 3 aspects were evaluated.Results Systemic symptoms were not found in 53 cases.49 cases were not found pain stimulated by hot and cold,night pain and spontaneous pain,visit found no tooth fracture and percussion pain,the success rate of treatment was 92.5%,3 cases among the remaining4 cases in2 years were gradually found the apicitis symptoms,accounting for 5.7% of the total number,1 cases in 5 days after operation appeared the symptoms of chronic pulpitis,accounting for 1.8% of the total number.Patient satisfaction survey results for the 53 patients,50 patients were satisfied with the operation times of costs and treatment time,visit during the operation,the satisfactory rate was 94.3%.Conclusion Minimally invasive tooth preparation methods on the adaptation of adult early pulpitis treatment effect is satisfactory.At the same time,compared with the treatment of tooth preparation method and the conventional root canal,it can decrease the cutting of the dental tissue,preserve the health of the teeth with maximum degree,which can achieve the purpose of minimally invasive treatment,and help to prolong the service
