电话随访对外伤脱位牙再植患者术后医从性和疗效的影响

摘　　要：目的探讨电话随访对外伤脱位牙再植患者术后医从性和疗效的影响。方法将粤北人民医院收治的外伤脱位牙患者126例随机平均分为观察组与对照组,2组患者均进行脱位牙再植入手术及常规护理,通知出院后复诊时间,观察组通过电话随访以了解患者术后情况并及时对患者进行指导。复诊时采用改良牙科焦虑量表评分（MDAS）评价2组患者焦虑程度和采用自行设计的医从性量表评价患者的医从性,比较2组患者复诊次数及复诊等候时间情况。结果观察组术后第7天MDAS评分显著低于对照组[（15.0±3.1）分vs.（16.2±3.3）分]、术后第14天的MDAS评分显著低于对照组[（10.5±3.6）分vs.（14.0±2.8）分],术后42天MDAS评分显著低于对照组[（8.2±1.9）分vs.（10.1±2.6）分],均P〈0.05;观察组术后第7天的医从性得分显著高于对照组[（31.9±4.8）分vs.（29.4±5.0）分],术后14天医从性得分显著高于对照组[（28.8±4.6）分vs.（22.4±5.7）分],术后42天医从性得分显著高于对照组[（28.3±4.2）分vs.（18.4±3.9）分];对照组患者术后15~42d复诊次数89次,平均（1.41±0.74）次,显著高于观察组的复诊次数59次,平均（0.94±0.29）次,对照组复诊总次数（3.73±1.83）次,显著高于观察组的（2.95±1.40）次,P〈0.05;对照组患者术后复诊1~7 d、8~14 d、15~42 d及总等候时间均显著长于观察组,P〈0.05。结论电话随访可有效改善外伤脱位牙再植入手术患者的牙科焦虑情况,提升其医从性,减少复诊次数及复诊等候时间,为患者提供了更加全面、便利的服务。Objective To explore the role of telephone follow-up in Traumatic dislocation after replantation. Methods 126 cases with Traumatic dislocation in northern Guangdong People＇ s Hospital were randomly divided into observation group and control group. All patients were treated with replantation and routine care. The patients were noticed referral time after hospital. The observation were added followed-up by telephone, Modified Dental Anxiety Scale score were used to evaluate Emotional status, the compliance behavior were assessed by a self-designed hospital. The referral times and ref- erral waiting time were compared. Results The MDAS scores at the first 7 d were significantly lower than those of the control group （ 15.0 ± 3.1 minutes vs. 16.2± 3.3 points ）, and so was the MDAS scores at the first 14 d, the score in the observation and the control group were （ 10.5 ± 3.6 points vs. 14.0 ±2.8 points）, the MDAS scores at 42nd were signifi- cantly lower than those of the control group（8.2± 1.9 points vs. 10. 1 ± 2.6 points） ,P 〈 0.05. Compliance behavior score at the 7th day in the observation group was significantly higher than that of the control group（ 31.9 ±4.8 points vs. 29.4 ± 5.0 points）, and the compliance behavior score at the 14th and 42nd in the observation group was significantly higher than that of the control group,the 14th score were（28.8 ±4.6 points vs. 22.4 ±5.7 points） ,and the 42nd score were（28.3 ±4.2 points vs. 18.4 ± 3.9 points） ,the control group was with 89 referral times,the average was 1.41± 0.74 times, and it was significantly higher than that of the average of 0.94± 0.29 time in the observation group. The total referral times in the control group was 3.73± 1.83, it was significantly higher than that of 2.95± 1.40 times in the obser- vation group. The referral waiting time at 1-7 d,8-14 d,15-42 d and total waiting time in the control group was significant- ly longer than that of the observation group, P 〈 0.05.
