胶囊内镜在不明原因消化道出血检查中的作用研究

摘　　要：目的:探讨胶囊内镜在不明原因消化道出血诊治中的价值及检查时机。方法:总结分析2010年12月至2014年04月我院158名接受胶囊内镜检查的不明原因消化道出血病变检出情况,根据出血表现形式及检查时机不同分组,统计分析各组患者不同检查时机的检出率及阳性率。结果:158名OGIB患者中检出病变117例,检出率74.05%,阳性80例,阳性率50.63%。显性出血组的阳性率与隐性出血组差异无统计学意义(P>0.05)。显性出血组的检出率随出血停止时间延长逐渐降低(P<0.05),阳性率于出血停止后3 d至2周最高(P<0.05)。隐性出血组2周内大便隐血阳性组检出率及阳性率均高于大便隐血阴性组(P<0.05)。结论:胶囊内镜对不明原因消化道出血具有较高的病变检出率。显性出血停止后3 d至2周,隐性出血大便隐血阳性2周内阳性检出率最高。Objective： To explore the value of capsule endoscopy（CE） in diagnosis and treatment of obscure gastrointestinal bleeding（OGIB） and the right checking time. Methods： 158 OGIB patients undergoing CE from December 2010 to April 2014 in our hospital were divided into obvious and occult bleeding groups, and the detectable rate and the positive rate were analyzed statistically at different checking time of CE. Results： Among158 patients, lesions were detected in 117 cases（74.05%）, whiles the positive lesions were detected in 80 cases（50.63%）. The positive rates between obvious bleeding group and occult bleeding group had no statistical difference（P〉0.05）. In obvious bleeding group, the detectable rate decreased with the extension of interval between checking time and bleeding stop time（P〈0.05）. The positive rate was the highest on 3 to 14 days after the bleeding stopped（P〈0.05）. In the occult bleeding group, the detectable rate and positive rate of the positive fecal oceult blood tests group were higher than those of the negative group within 2 weeks（P〈0.05）.Conclusion： CE has a high detectable rate in OGIB. The checking time of the highest positive detection rate is from 3 to 14 days after the obvious bleeding stopped and within 2 weeks of the positive fecal occult blood tests.
