口服普萘洛尔治疗婴幼儿头颈部血管瘤的临床研究

摘　　要：目的：评价口服普萘洛尔治疗婴幼儿头颈部血管瘤的临床疗效和安全性。方法：2009年3月~2013年12月,45例婴幼儿头颈部血管瘤患者在徐州市中心医院口腔颌面外科完成普萘洛尔治疗,按1.0~1.5 mg/kg,3次/d,连续服用。1次/月动态观察和记录血管瘤大小、质地及颜色的变化,深部或混合性血管瘤则行彩超检查记录体积变化,随时处理治疗过程中出现的不良反应。按4级评分法对近期疗效进行评价。结果：45例患儿平均口服普萘洛尔7.6个月,疗效：Ⅰ级（差）2例,Ⅱ级（中）7例,Ⅲ级（好）21例,Ⅳ级（优）15例。45例均未发生严重不良反应。5例复发,再次服用普萘洛尔后仍有效。结论：口服普萘洛尔治疗婴幼儿头颈部血管瘤近期疗效好,尤对体积较大的血管瘤疗效显著,不良反应轻微,但治疗时间尚需进一步研究。Objective：To evaluate efficacy and safety of propranolol for the treatment of infantile hemangiomas（IH）on the head and neck region.Methods：Between March 2009 and Decmber 2012,oral propranolol at a dose of1.0-1.5mg/kg three times per day was administered to 45 infants with hemangiomas in xuzhou central hospital.The changes of the tumor size,colour and texture were monitored and recorded at a regular interval.At the same time,size of the deep or mixed hemangiomas was documented by ultrasound echocolor Doppler.The adverse effects after medication were observed and managed accordingly.Degree of clinical improvement in overall apperance was rated as follow：Ⅰ（poor：﹤25%）,Ⅱ（moderate：26-50%）,Ⅲ（good：51-75%）,orⅣ（excellent：﹥75%）.Results：The average propranolol treatment duration for the 45 patients was 7.6 months,The overall response was poor in 2patients,moderate in 7patients,good in 21 patients,and excellent in 15 patients.No serious adverse events were encountered.A second course of propranolol readily controlled the recurrence in 5patients（11.1%）.Conclusion：Oral propranolol for IH,yielded excellent results with very goog clinical tolerance,especially for larger hemangiomas.The optimal duration of the treatment remains to be defined by long-term observation.
