不同疗程无抽搐电休克治疗对精神分裂症患者认知功能的影响

摘　　要：目的探讨不同疗程无抽搐电休克治疗(MECT)对精神分裂症患者认知功能的影响。方法将符合条件的精神分裂症患者随机分为两组,N1组给予6次MECT治疗(n=46),N2组给予12次MECT治疗(n=44)。在治疗前后分别进行威斯康星卡片分类试验(WCST-M)、Stroop测验、连线测试(TMT),并行阳性与阴性症状量表(PANSS)评定。结果 1与基线时相比,MECT治疗后两组PANSS总分及各分量表评分均明显下降(P<0.05),且N2组比N1组的下降更明显(P<0.05)。2与基线时相比,两组MECT治疗后WCST-M的总测验数、正确反映数、持续错误数、非持续错误数及分类完成数差异有统计学意义(P<0.05)。而且N2组的WCST-M总测验数、正确反映数、持续错误数及分类完成数均明显优于N1组(P<0.05)。3与基线时相比,MECT治疗后两组Stroop CW总反映时间、CW错误数、C总时间、C总错误数测验及TMT-A和B差异有统计学意义(P<0.05)。MECT治疗后相比,N2组CW总反映时间、C总错误数测验及TMT-A和B与N1组差异有统计学意义(P<0.05)。结论 MECT治疗后,精神分裂症患者的认知功能有一定程度的改善。ABSTRACT：Objective To investigate the effect of different periodical modified electroconvulsive therapy （MECT）on the cognitive functions of patients with schizophrenia.Methods Ninty-eight patients with schizophrenia were randomly divided into two groups：group N1（n =46）and group N2 （n =44）.Patients in group N1 received 6 periods of MECT and patients in group N2 12 periods of treatment.Before and after the treatment,Wisconsin Card Sorting Test（WCST）,Stroop test,trail making test A and B and positive and negative syndrome scale（PANSS）were performed on each patient to assess the cognitive functions.Results Compared with the baseline,the total scores of PANSS and other scores were significantly reduced by MECT treatment in both groups,especially in group N2 （P 〈0.05）;the results of WCST,TMT and Stroop test improved markedly after MECT treatment in both groups, especially in group N2 （P 〈0.05）.Compared with group N1,the WCST,TMT and Stroop test in N2 group were all better than those of group N1 （P 〈0.05）.Conclusion MECT treatment significantly improves the cognitive functions of patients with schizophrenia.
