有晶状体眼前房型人工晶体植入术矫治超高度近视远期效果观察

摘　　要：目的观察有晶状体眼前房型人工晶体植入术矫治超高度近视远期效果。方法对屈光度为-12.0^-22.0 D的21例35眼超高度近视,采用Phakic-6H有晶状体眼前房型人工晶体植入手术矫治,术后连续观察6年以上。结果术后6年的裸眼视力0.3~1.2,优于术前最佳矫正视力者14眼(40.0%);最佳矫正视力0.4~1.2,超过术前最佳矫正视力者27眼(77.1%),无最佳矫正视力下降者。术后6年屈光状态为0.00^-3.00 D。术前、术后6年平均眼压分别为(14.80±2.33)mm Hg、(15.24±2.75)mm Hg,对比物差异无统计学意义,p>0.05。内皮细胞计数术前为(3 110±259)个/mm2,术后6年为(2 746±365)个/mm2,差异无统计学意义,p>0.05。术后早期2眼出现眼压升高,1眼前房出血,均经药物治疗后恢复。结论有晶状体眼前房型人工晶体植入术是一种有效矫治超高度近视方法,且远期治疗效果显著。但仍要严格掌握适应症,术后长期随访,特别是跟踪角膜内皮细胞的变化是必要的。Objective To observe a lens front room super high myopia iol implantation correction of long - term outcome. Methods 21 cases of 35 high myopia to diopter - 12.0D to -22. OD, by Phakic -6H phakic ante- rior chamber intraocular lens implantation therapy, were continuously observed more than six years. Results After 6 years of O. 3 1.2, naked eye eyesight is better than that of preoperative 14 eyes （ 40.0% ） , and achieved the best corrected visual acuity ; Best corrected visual acuity 0.4 - 1.2 is better than preoperative best corrected vision 27 eyes （ 77.1% ）. There is no best corrected vision loss. Refractive status after 6 years of 0.00 D - - 3.00 D. Before surgery, 6 years the average IOP were （ 14.80 ± 2.33 ） mmHg, （ 15.24 ± 2.75 ） mmHg. There are no significant difference （p 〉 0.05 ）. Endothelial cell count before surgery is （ 3 110 ± 259 ） a / mm2, after 6 years （2 746 ± 365 ） a./mm2, the difference was not statistically significant （p 〉 0.05 ）. 2 Early postoperative intraocular pressure, a hyphema, were recovered after drug treatment. Conclusion Anterior chamber phakic intraocular lens implantation is an effective method of correcting high myopia, and the long - term treatment effect is remarkable. But still strictly controlled indications, postoperative long - term follow - up, in particular to track changes in corneal endothelial cells is necessary.
