Relevance of Non-Enzymatic Glycosylation with Left Ventricular Diastolic Function in Elderly Patients with Type 2 Diabetes

摘  要:目的:研究老年糖尿病患者羧甲基赖氨酸(CML)水平与血管硬化及左心室舒张功能减退的关系。方法:选取2013年12月-2014年8月本院住院的老年糖尿病患者116例,应用ELISA法检测血浆CML水平;多普勒超声心动仪测量左心室舒张早期血流速度(E)、舒张晚期血流速度(A),并计算出E/A,组织多普勒测量舒张早期运动速度峰值(E’),计算舒张早期血流速度与瓣环运动速度比值E/E’;用全自动动脉硬化测量仪测定动脉搏波传导速度(PWV)。结果:单因素分析显示,CML、年龄与ba PWV、E/E’均呈正相关(P<0.05);与E/A呈负相关(P<0.05)。多元回归分析显示,CML与ba PWV、E/E’呈正相关(P<0.01);CML与E/A呈显著负相关(P<0.01)。结论:老年糖尿病人群CML水平的升高导致左室舒张功能下降;降低糖基化终末产物的生成可以作为改善患者左室舒张功能的一种干预手段。Objective:To observe the relevance of serum carboxymethyl with arterial stiffness and left ventricular diastolic function in elderly patients with type 2 diabetes.Method:116 elderly patients with type 2 diabetes from December 2013 to August 2014 in our hospital, and ELISA was applied to measure serum CML. The maximal velocity of mitral flow E peak and A were tested by two-dimensional echocardiography. The maximal speed of mitral flow E' was measured by tissue Doppler.The E/A and E/E' were calculated. The pulse wave velocity(PWV) was measured by Auto-survey atherosclerosis apparatus.Result:CML and age were positively correlated to ba PWV and E/E' by the univariate analysis(P〈0.05). CML and age were negatively correlated to E/A(P〈0.05).The multiple regression analysis showed CML was positively correlated to ba PWV and E/E'(P〈0.01),while it was negatively correlated to E/A(P〈0.01).Conclusion:CML is associated with increased arterial stiffness and decreased diastolic function in elderly patients with type 2 Diabetes.An treatment to decreasing advanced glycation end-product(AGEs)may be used to lower arterial stiffness and improve diastolic function.pulse wave velocity(PWV).
