泮托拉唑对胃食管反流伴声嘶的疗效分析

摘　　要：目的:探讨泮托拉唑治疗胃食管反流病(GERD)伴声嘶的临床疗效。方法:41例由反流疾病问卷(RDQ)和胃镜确诊的GERD伴声嘶的患者,行电子喉镜检查和反流症状指数量表(RSI)、反流检查计分量表(RFS)、嗓音症状量表(Voi SS)评估,将其中RSI评分>13分且RFS评分>7分者诊断为喉咽反流(LPR)。所有患者给予泮托拉唑钠肠溶胶囊,2次/d,疗程6个月,并于治疗第3、6个月复诊时评价RDQ、RSI和Voi SS。结果:比较完成随访的患者,治疗结束时RDQ、RSI和Voi SS均较治疗前均降低(P<0.05),其中RDQ第6个月与第3个月相比仍有明显下降,差异有统计学意义(P<0.05),而RSI与Voi SS在第3、6个月之间差异无统计学意义(P>0.05);相关分析表明,RSI与Voi SS具有较强相关性(r=0.526,P=0.003),但RDQ与RSI、Voi SS无明显相关(P>0.05)。结论:泮托拉唑对GERD伴声嘶患者疗效确切,但确定合适的疗程需要更多的研究。Objective： To investigate clinical efficacy of pantoprazole for gastroesophageal reflux disease（GERD） with hoarseness. Method：41 patients were diagnosed of GERD and hoarseness by reflux disease questionnaire（RDQ） and endoscopy. Electronics laryngoscopy, reflux symptom index scale（RSI）, reflux finding score（RFS） and voice symptom scale（Voi SS） were assessed after diagnosed.Those would be diagnosed as laryngopharyngeal reflux（LPR） by RSI13 scores and RFS7 scores. All patients were given pantoprazole sodium enteric-coated capsules, twice daily for 6 months.RDQ, RSI and Voi SS would be evaluated at the third month and the sixth month post-treatment. Result：Patients completed the follow-up. At the end of treatment RDQ,RSI and Voi SSwere lower than per-treatment,the difference was statistically significant（P〈0.05）. RDQ still decreased significantly at the sixth month compared to the third month and the difference was statistically significance（P〈0.05）.There was no differences in RSI and Voi SS between the third month and the sixth month（P〉0.05）.It was strong correlation between RSI and Voi SS（r= 0.526,P=0.003） but no significant correlation between RDQ and RSI, Voi SS（P〉0.05）.Conclusion： Treatment of GERD with twice daily pantoprazole resulted in improved subjective voice for patients with hoarseness, but the appropriate course of treatment require more research.
