放疗前HB浓度对鼻咽癌调强放疗敏感程度和预后的评估价值

摘　　要：目的:探讨放疗前血红蛋白(HB)浓度对鼻咽癌(NPC)调强放疗敏感程度和预后的评估价值。方法:选取2006年2月-2010年5月本院确诊并通过调强放疗治疗的NPC患者231例,依据HB浓度分为正常组、轻度组、中度组、重度组,应用Spearman分析法分析NPC调强放疗局部控制效果与HB浓度的关系,统计分析所有患者放疗后病变部位局部控制情况和1、3、5年内生存情况。结果:放疗后在总局部控制率方面:正常组>轻度组>中度组>重度组,差异均有统计学意义(F=5.89,P<0.05);Spearman分析法结果显示,NPC调强放疗局部控制效果与HB浓度呈现负相关(r=-0.542,P=0.003);在各时段生存率方面:正常组>轻度组>中度组>重度组,差异均有统计学意义(P<0.05)。结论:放疗前HB浓度低使NPC患者血液供氧能力低,致使癌细胞处于乏氧状态,导致放疗对癌细胞灭杀能力下降,降低患者预后水平。Objective：To discuss the evaluation of value before radiotherapy hemoglobin（HB） concentration sensitive degree of intensity modulated radiation therapy for nasopharyngeal carcinoma（NPC） and prognosis.Method：231 patients with NPC were selected according to the HB, all patients were divided into the concentration, mild, moderate and severe group, Spearman analysis method was used to analyze NPC local control effect and HB concentration, the relationship between pathological changes of statistical analysis of all patients after radiotherapy local control and survival situation within 1, 3, 5 years.Result：After radiotherapy in total local control aspects： the normal group mild group moderate heavy group（F=5.89,P〈0.05）.Intensity modulated radiation therapy Spearman analysis method, according to the results of NPC local control effect and HB concentration appear negative correlation（r=-0.542,P=0.003）.In each period survival aspects： normal group mild groupmoderateheavy group（P〈0.05）.Conclusion：Low NPC patients before radiotherapy HB concentration low blood oxygen, causing cancer cells is in a state of lack of oxygen, resulting in a decline in radiation therapy for cancer cell killing ability and the prognosis of patients with lower levels.
