C反应蛋白与脑梗死急性期神经功能缺损及预后的相关性分析

摘　　要：目的探讨C反应蛋白和急性脑梗死患者神经功能缺损程度以及预后的相关性。方法以我院神经内科2012-01—2013-12收治的180例急性脑梗死患者为试验组,同期体检健康者180例为对照组,采用速率散射比浊法测定急性脑梗死发作24h、3d、1周、2周血清CRP水平,分析其和神经功能缺损和预后的相关性。结果试验组急性发作24h、3d、1周、2周内血清CRP水平明显高于对照组(P<0.05),试验组CRP水平和神经功能缺损程度呈正相关(P<0.05);脑梗死患者CRP水平和梗死灶大小呈正相关(P<0.05);127例神经功能恢复良好,53例恢复不良,CRP水平和mRS评分呈正相关(r=0.277,P<0.05),恢复良好患者Barthel指数评分明显高于恢复不良患者(P<0.05)。结论血清CRP水平对急性脑梗死早期诊断、病情评估以及预后评价等方面具有重要意义,值得临床推广。Objective To investigate the correlation between C-reactive protein and neurological deficits and prognosis of patients with acute cerebral infarction.Methods 180 patients with acute cerebral infarction in our hospital from January 2012 to December 2013 were considered as test group,and 180 healthy persons were selected as control group.The serum CRP level of two groups were detected by rate nephelometry method 1d,3d,1week and 2week after acute cerebral infarction attack to analyze the correlation between neurologic function impairment and prognosis.Results The serum CRP level of test group was significantly higher than that of control group（P〈0.05）at the same time,and there was positive correlation between CRP level and neurologic function defect.The serum CRP level of patients with acute cerebral infarction was positive correlation with infarction volume.There were 127 cases of good recovery and 53 cases of poor recovery.The Barthel index score of patients with good recovery was higher than that of patients with poor recovery.Conclusion The serum level of CRP is important in the early diagnosis,disease assessment and prognosis evaluation of patients with acute cerebral infarction,which is worthy of clinical application.
