运气学说在古代医案中的临床应用研究

摘　　要：目的探讨古代医家应用运气诊疗疾病的经验和规律,为运气学说指导现代临床提供参考。方法选择10部有代表性的医案类著作,检索运气医案,建立运气医案的数据库,对数据进行整理、统计和分析,总结疾病病种、运气特征、病机、辨证、治疗和预后等方面的规律。结果共纳入28个医案,包括17种病证。发病年份的岁运主要集中在火运之年,客气的司天之气和在泉之气以阳明燥金和少阴君火多见。疾病的八纲属性主要为阳证、里证、热证和实证。在常规的辨证论治基础上,结合运气学说有助于认识疾病病因病机,提高疗效,判断预后。结论古代运气医案中运气理论的应用对部分疾病的发病、证型、治疗和预后有其重要影响,古代医家应用运气学说诊疗疾病的经验值得现代中医临床借鉴。Objectives To explore the application patterns of Yunqi theory by ancient TCM physicians in the differentiation and treatment of disease,in the hope of providing reference for current clinical practice.Methods Medical records related to the Yunqi theory were searched and identified in ten representative books of ancient TCM medical records.The characteristics of Yunqi medical records were extracted,including disease patterns,Yunqi characteristics,pathogenesis,syndrome differentiation,treatment,and prognosis.The data were summarized and analyzed,so as to explore the role of Yunqi in ancient TCM clinical practice.Results Twenty-eight medical records involving seventeen disease patterns were included.The time when diseases were prevalent tended to be Fire-yun year,while the Guest qi mainly manifested as Yang-ming in the first half of the year and Shao-yin in the second half of the year.In terms of differentiation of the eight principles,most diseases were yang,interior,heat and excess patterns.The combination of conventional TCM syndrome differentiation and Yunqi theory was helpful to understand the etiology and pathogenesis of diseases.Therefore,it could improve therapeutic efficacy and help make accurate prognosis.Conclusion The ancient medical records involving the Yunqi theory have shown that Yurnqi theory exerted great effect on the etiology,pathogenesis,syndrome differentiation,treatment,and prognosis of some diseases.Such experience as what ancient TCM doctors had in using Yunqi theory in diagnosis and treatment of disease is still of value to the current clinical practice of TCM.
