摘　　要：目的探讨腔镜手术对直肠癌患者免疫平衡及CD4+CD45RA+T细胞和CD4+CD45RO+T细胞表达的影响。方法67例直肠癌患者中,33例拟进行腹腔镜手术,设为L组,34例拟进行开腹手术,设为O组。观察术前1 d、术后2 d、术后7 d患者Th1、Th2、CD4+CD45RA+T、CD4+CD45RO+T细胞表达水平。结果 L组患者术前和术后Th1、Th2、CD4+CD45RA+T、CD4+CD45RO+T细胞表达无显著差异(P>0.05)。O组术后2 d、术后7 d患者Th1细胞表达水平显著下降(P<0.01),Th2细胞表达显著升高(P<0.01);CD4+CD45RA+T细胞的表达显著升高(P<0.01),CD4+CD45RO+T细胞的表达显著降低(P<0.05)。结论腹腔镜手术较开腹手术对直肠癌患者的免疫平衡、CD4+CD45RA+T细胞和CD4+CD45RO+T细胞表达的影响小,对机体细胞免疫功能干扰较轻。Objective To explore the effects of laparoscopy-assisted rectal cancer surgery on immune balance and expression of CD4^ ＋ CD45RA^＋ T and CD4^ ＋ CD45RO^＋ T cells in patients with rectal cancer. Methods A total of 67 patients with rectal cancer were divided into group L （ n = 33） and group 0 （ n = 34）, wish laparoseopy-assisted surgery and laparotomy surgery, respectivdy. Thl cell, Th2 cell, ratio of Thl/Th2, CD4^＋ CD45RA^＋ T cell, CD4^＋ CD45RO^＋ T cell and ratio of CD4^＋ CD45RA^＋ T/CD4^＋ CI345RO^＋ T were detected at 1 d prior to, 2 d and 7 d after operation. Results In the group L, there was no significant difference in levels of Thl, Th2, CD4 ^＋ CD45RA^＋ T, CD4^＋ CD45RO^＋T before and after surgery （P〉0.05）. In the group O, The level of Thl cell was decreased at 2 and 7 days postoperatively （P〈 0.01 ） , while the level of Thl cell was increased （P 〈 0.01 ） ; The level of CD4^ ＋ CD45RA^＋ T cell was increased 2 and 7 days after operation （ P 〈 0.01 ）, while the level of CD4^＋ CD45RO^＋ T cell was reduced （P 〈 0.05）. Conclusion Compared with laparotomy, laparoscopy-assisted surgery has less effect on immune balance and expression of CD4^＋ CD45RA^＋ T cell and CD4 ^＋ CD45RO ^＋ T.
