胃癌超声内镜术前分期与组织中基质金属蛋白酶-2和组织基质金属蛋白酶抑制因子-2表达的关系

摘　　要：目的探讨内镜超声对胃癌的术前分期及其与肿瘤转移相关基因表达的关系。方法 44例胃癌患者术前行胃镜和超声内镜(EUS)检查并进行术前分期,同时运用ELASA法检测不同部位组织基质金属蛋白酶-2(MMP-2)及组织基质金属蛋白酶抑制剂-2(TIMP-2)的表达水平,并与术后病理分期比较。结果 EUS对胃癌侵犯深度判断的总准确率为81.82%,对淋巴结转移的总准确率为84.09%。胃癌组织及正常组织中MMP-2及TIMP-2的浓度有明显差异,表达水平与胃癌浸润程度、EUS分期、淋巴结转移密切相关(P<0.05),但胃癌组织中TIMP-2的浓度虽然随超声内镜TN分期加深明有下降趋势,但却无显著统计学差异。结论 1EUS对胃癌的分期具有其分子生物学改变的基础。2胃癌患者组织中MMP-2及TIMP-2水平对EUS术前分期有参考价值,若MMP-2及TIMP-2表达水平显著增高者,术前EUS检查时,应注意癌肿是否已达到或超过浆膜层。Objective To investigate the relationship between the staging of endoscopic ultrasonography（ EUS）and expression of carcino-ma metastasis associated genes in the patients with gastric cancer. Methods 44 patients with gastric cancer were diagnosed by electric gastroscopy and EUS. The preoperative staging were measured by EUS and compared by pathologic staging. MMP-2 and TIMP-2 expression in cancers were compared by normal tissues,EUS staging and pathologic staging. Results The total accuracy rate of EUS in estimating invasive depth of gastric cancers was 81. 82%,while that of lymphatic metastasis was 84. 09%. The expression level of MMP-2 and TIMP-2 in cancer tissue is different with normal tissue and the expression is closely correlative with invasive degree,the EUS staging and lymphatic metastasis（ P 〈0. 05）. The ex-pression of TIMP-2 in cancer tissue declined with EUS staging,but have no statistical significance. Conclusion The changes of EUS staging have the molecular biologic basis. The level of MMP-2 and TIMP-2 in tissues have the reference value for preoperative staging of EUS in pa-tients with gastric cancer.
