结肠癌术后辅助化疗对血Caspase-3、Survivin、CD44v6表达的影响

摘　　要：目的探讨结肠癌术后辅助化疗对外周血半胱氨酸天冬氨酸特异性蛋白酶3(Caspase-3)、存活素(Survivin)mRNA及黏附分子CD44拼构变异体6(CD44v6)表达的影响及其意义。方法接受根治性切除术的TNM分期Ⅱ期结肠癌患者60例,按术后是否进行辅助化疗分为未化疗组30例,化疗组30例。化疗组化疗方案为奥沙利铂/亚叶酸钙/5-氟尿嘧啶,共化疗6个疗程。比较两组患者术前1 d、术后6个月外周血Caspase-3、Survivin、CD44v6的表达水平。应用RT-PCR检测Caspase-3和Survivin表达,应用流式细胞仪检测CD44v6表达。结果术前两组外周血Survivin、Caspase-3、CD44v6表达水平比较,差异无统计学意义(P>0.05)。术后6个月,两组外周血Survivin、CD44v6表达水平较术前明显降低(P<0.05),且化疗组下降更明显(P<0.05)。术后6个月,两组外周血Caspase-3表达水平较术前明显升高(P<0.05),且化疗组升高更明显(P<0.05)。结论结肠癌术后辅助化疗可能通过细胞凋亡机制抗肿瘤复发,且术后辅助化疗具有抑制肿瘤转移的作用。Objective To explore the changes and significance of Caspase-3,Survivin and CD44v6 in peripheral blood of patients treated with adjuvant chemotherapy after colectomy. Methods Sixty patients with stage Ⅱcolon cancer who had undergone colectomy were divided into chemotherapy group（ n = 30） and non-chemotherapy group（ n = 30） according to whether they received adjuvant chemotherapy after colectomy or not. The chemotherapy group was treated with chemotherapy regimen of oxaliplatin,calcium folinate and 5-fluorouracil for six courses. The expression levels of Caspase-3,Survivin and CD44v6 in peripheral blood one day before operation and six months after operation were compared between two groups.RT-PCR was used to detect the expressions of Caspase-3 and Survivin,while flow cytometry was used to detect the CD44v6 expression. Results There was no significant difference in the expression levels of Survivin,Caspase-3 and CD44v6 between two groups before operation（ P〉0. 05）. Six months after operation,the expression levels of Survivin and CD44v6 in peripheral blood decreased significantly in two groups in contrast with those before operation（ P〈0. 05）,and the decrease in the chemotherapy group was more significant（ P〈0. 05）. Six months after operation,the expression level of Caspase-3 in peripheral blood increased significantly in two groups compared with the preoperative（ P〈0. 05）,and the increase in the chemotherapy group was more significant（ P〈0. 05）. Conclusion Adjuvant chemotherapy after colectomy might play an anti-tumor role in colon cancer via cell apoptosis. It might inhibit tumor metastasis.
