摘  要:目的探讨Roux-en-Y胃旁路术保护糖尿病大鼠肾脏功能的可能机制。方法采用链脲佐菌素(streptozotocin,STZ)制作糖尿病大鼠模型,经筛选后随机分为糖尿病组(DG组,n=8)、假手术组(SOG组,n=8)、手术组(OG组,n=6)及正常对照组(NCG组,n=8);术后8周观察各组血糖、血尿素氮、血肌酐以及蛋白激酶C(protein kinase C,PKC)-β和蛋白激酶A(protein kinase A,PKA)在肾脏的表达变化。结果手术组血糖、血尿素氮(blood urea nitrogen,BUN)和血肌酐(creatinine,CR)均明显低于糖尿病组和假手术组(P<0.05),PKC-β的m RNA以及蛋白表达水平均较糖尿病组和假手术组低(P<0.05)。PKA的m RNA和蛋白表达水平则较糖尿病组和假手术组高(P<0.05),而手术组与正常对照组无明显差异(P>0.05)。结论 Roux-en-Y胃旁路术后大鼠PKC-β在肾脏表达降低;PKA在肾脏表达增高,使肾脏氧化应激和炎症反应等降低,避免了糖尿病肾损害进一步发展,有效的保护了肾脏功能。【Objective】 To investigate the mechanism of the protective effects of Roux-en-Y gastric bypass surgery on kidney function in diabetes mellitus rats. 【Methods】Diabetes mellitus animal models were induced by intraperitoneal injection of streptozotocin(STZ) and a high-fat diet. Diabetic rats were divided into three groups randomly: diabetes group(DG, n =8), sham operation group(SOG, n =8), operation group(OG, n =6). Another 8 normal SD rats were used as the normal control group(NCG, n =8). The blood glucose, blood urea nitrogen, serum creatinine were measured before and 8 weeks after the operation in each group. The expressions of PKA and PKC-βin renal tissue were measured by Real Time PCR(RT-PCR) and Western blot. The expression of fibronectin and type IV collagen in renal tissue was investigated by immunohistochemical staining.【Results】The blood glucose, blood urea nitrogen and serum creatinine of OG group were significantly lower than DG group and SOG group(P〈0.05). The m RNA and protein expression of PKC-β in OG group were lower than DG group and SOG group(P〈0.05). The m RNA and protein expression of OG were significantly higher than DG group and SOG group(P〈0.05). OG group had no significant difference from NCG group(P〉005).【Conclusion】Roux-en-Y gastric bypass surgery can reduce the expression of PKC-β and increase the expression of PKA in diabetes rats; reduce renal oxidative stress and inflammatory reaction in renal tissue; and protect the renal function ultimately.
