根充联合改良塑化术治疗老年牙髓病和根尖周病的效果评价

摘　　要：目的探讨不同方法治疗老年磨牙牙髓病和根尖周病的临床效果。方法回顾性分析本院2011年3-12月口腔科就诊的磨牙牙髓病、根尖周病的老年人患者100例,共有120颗患有牙髓病、慢性根尖周炎的磨牙。将其随机分成对照组48例和观察组52例,每组60颗患牙。对照组按常规根充进行治疗,观察组采用根充联合改良塑化术治疗,采用根尖周指数（PAI）评定X线片并结合临床检查,对疗效进行评价,随访2年。结果对照组患牙的恰填率为81.67%,观察组为95.00%,两组患牙的恰填率比较,差异有统计学意义（χ^2=9.89,P〈0.01）;对照组术后1周的患牙疼痛率为20.83%,观察组为11.54%,两组比较,差异有统计学意义（χ^2=7.40,P〈0.01）;两组患者除对照组1例患者失访外,均随访2年,临床检查均无松动、叩痛、无漏管,X线检查根尖周无阴影;术后2年随访观察,观察组的总有效率为95.0%,对照组为86.7%,两组比较,差异有统计学意义（P〈0.05）。结论根充联合改良塑化术治疗老年人磨牙牙髓病和根尖周病简化了技术操作,减少了医生和患者的体力消耗,提高了临床治疗效果,提高了患者满意度,可满足临床需要,值得临床推广应用。Objective To explore the clinical effect of the different methods treating senile molar pulp disease and periapical disease. Methods 100 senile patients with molar pulp disease and periapical disease treated in department of stomatology from March to December 2011 in our hospital were retrospectively analyzed.There were 120 molar teeth with pulpal disease and chronic periapical disease.The patients were divided into the control group（n=48） and the observation group（n=52）,there were 60 sick teeth in each group.The control group was treated by routine root canal filling,the observation group was given the root canal filling combined with improved resinifying therapy.PAI was used to evaluate X-ray film,and combined with clinical examination to evaluate the curative effect,the patients were followedup for 2 years. Results The just filling rate of sick tooth in the control group was 81.67%,the observation group was95.00%,and there was a statistical difference in the two groups（χ^2=9.89,P〈0.01）;the pain rate of sick tooth after 1 week operation in the control group was 20.83%,the observation group was 11.54%,and there was a statistical difference in the two groups（χ＆2=7.40,P〈0.01）;after follow-up observation of 2 years,the effective rate of the observation group was95.0%,the control group was 86.7%,there was a statistical difference in the two groups（P〈0.05）;the patients in the two groups except for 1 patients in the control group were lost of follow-up,clinical examination showed no loosening,percussion pain,no leakage tube,X-ray periapical without shadow. Conclusion Root canal filling combined with improved resinifying therapy treating senile pulpal and periapical disease simplifies the technical operation,reduce the consumption of the doctor and the patients′ physical strength,improve the clinical treatment effect,improve the satisfaction degree of patients,which can meet the clinical needs,it is worthy of clinical promotion and application.
