腹腔循环热灌注化疗序贯深部热疗治疗恶性腹膜积液的临床研究

摘　　要：目的探讨腹腔循环热灌注化疗序贯深部热疗治疗恶性腹腔积液的疗效。方法将我院2013年01月~2014年06月收治的65例恶性腹腔积液患者随机分成两组：观察组,第一天进行腹腔循环热灌注化疗（每次顺铂40~60 mg）,第2、5 d序贯腹部深部热疗;对照组,单纯给予顺铂腹腔内灌注化疗。每周一次,2次为一疗程,2个疗程后评价疗效及不良反应。结果观察组恶性腹腔积液总有效率（RR）为84.4%,;对照组总有效率为34.4%。差异有统计学意义（P〈0.05）。观察组的生活质量评价改善18例（56.3%）,稳定10例（31.2%）,下降4例（12.5%）,对照组中改善8例（25%）,稳定例18（56.3%）,下降例6（18.9%）;观察组生活质量改善优于对照组,差异有统计学意义（P〈0.05）。两组毒副反应差异无统计学意义（P〉0.05）有可比性。结论腹腔循环热灌注化疗序贯深部热疗治疗恶性腹腔积液疗效显著,毒副反应低,生活质量明显改善。Objective To investigate the efficacy of peritoneal circulatory hyperthermia perfusion chemotherapy sequential deep thermotherapy in the treatment of malignant pleural effusion. Methods 65 patients with malignant pleural effusion from January 2013 to June 2014in our hospital were randomly divided into 2 groups. The control group was treated with simple perfusion chemotherapy,the observation group underwent abdominal perfusion chemotherapy dl,and sequential deep thermotherapy treatment d2, 5, 1 time a week, 2 times as a course , The responses of curative effects and adverse reactions were observed after two courses . Results The total efficiency of observation group was 84.4%,significantly higher than the control group, which was 34.4% （P〈0.05）. The quality of life were improved in 18 cases （56.3%）,stable in 10 （31.2%）and decreased in 4（12.5%） of observation group, while improved in 8 cases （25%）,stable in 18 （53.6%）and decreased in 6（18.9%） of control groupThe observation group＇s quality of life was superior to the control group （P〈0.05） . Conclusion Peritoneal circulatory hyperthermia perfusion chemotherapy sequential deep thermotherapy in the treatment of malignant pleural effusion shows better, curative effects with few adverse reactions and significantly improved the paition＇s quality of life.
