摘 要:目的探讨高频振荡通气(HFOV)联合骨髓间充质干细胞(MSCs)对内毒素诱发急性肺损伤模型兔炎症反应的影响。方法家兔34只,2只用于制备骨髓间充质干细胞,剩余32只随机分为4组:肺损伤组、HFOV组、MSCs移植组和HFOV+MSCs移植组,每组8只。全骨髓培养法体外培养兔MSC,传至第3代备用。通过向气管内滴注内毒素建立急性肺损伤/急性呼吸窘迫综合征模型。分别在干预后24 h时间点,采用ELISA法检测各组肺组织匀浆中IL-lβ、TNF-α和IL-10含量;采用RT-PCR法检测各组肺组织IL-lβ、TNF-α和IL-10m RNA的表达。结果 HFOV组和MSCs组,肺组织IL-1β与TNF-α水平及其m RNA表达降低,而IL-10水平及其m RNA表达升高,分别与肺损伤组相比,差异有统计学意义(P<0.05);联合HFOV+MSCs组中肺组织IL-1β与TNF-α水平及其m RNA表达最低,IL-10水平及其m RNA表达最高,分别与单一HFOV组和MSCs组比较,差异有统计学意义(P<0.05)。结论 HFOV联合MSCs能显著降低ALI/ARDS体内促炎因子IL-1β和TNF-α的水平及其m RNA的表达,升高抗炎因子IL-10的水平及其m RNA的表达,对ALI/ARDS具有抗炎保护作用,优于单一方法,具有协同作用。[ Objective ] To investigate the influence of high frequency oscillatory ventilation combined with bone marrow mesenehymal stem cell transplantation on inflammatory response in endotoxin-induced acute lung injury in rabbits. [ Methods ] A total of 34 rabbits were used in this study. Two rabbits were utilized to culture BMSCs. 32 rabbits were randomly assigned to four groups, acute lung injury group, high frequency oscillatory ventilation (HFOV) group, MSCs transplantation group and HFOV combined with MSCs transplantation group (n =8). Rabbit BMSCs were isolated and cultured by the whole bone marrow culture method. At the third passage, BMSCs were harvested for use. Endotoxin was infused into the trachea to establish models of acute lung injury/ARDS. 24 h after intervention, Interleukin-1β (IL-1β), tumor necrosis factor-α (TNF-α and interleukin-10 (IL-10) levels and their mRNA expres- sion in the lung tissue of each group were measured by ELISA and RT-PCR. [Results] IL-1β, TNF-cdevels and their mRNA expression in the lung tissue of HFOV group and MSCs groups were decreased, while IL-10 levels andmRNA expression were increased in HFOV group or MSCs group compared acute lung injury group, the differences were statistically significant (P 〈0.05); IL-1β, TNF-α levels and their mRNA expression in the lung tissue of the HFOV+MSCs group were the lowest, while IL-IO levels and mRNA expression was the highest, HFOV+MSCs group was compared with HFOV group and MSCs group respectively, the differences were statistically significant(P 〈0.05). [Conclusion] HFOV combined with MSCs transplantation has anti-inflammatory protective effect on ALI/ARDS through significantly reducing proinflammatory cytokines IL-1β and TNF-α levels and its mRNA expression, and increasing anti-inflammatory cytokine IL-10 levels and its mRNA expression in vivo. It has a synergistic effect, which was better than a single method.
