子宫内膜异位症相关卵巢癌的临床分析

摘　　要：目的探讨子宫内膜异位症相关卵巢癌的临床特点。方法回顾性分析45例子宫内膜异位症相关卵巢癌患者(研究组)的临床特点,与同期收治的88例卵巢浆液性腺癌患者(对照组)进行比较。结果研究组发病年龄≤50岁患者所占比例为55.56%,对照组为30.68%,差异有统计学意义(P<0.05)。研究组FIGO分期早期(Ⅰ+Ⅱ期)患者所占比例为75.56%,对照组为21.59%,差异有统计学意义(P<0.05)。研究组中血CA125值正常患者所占比例为35.56%,对照组为6.81%,差异有统计学意义(P<0.05)。研究组中同时合并有子宫内膜样癌患者8例,占17.78%,对照组中无合并有子宫内膜样癌的患者,差异有统计学意义(P<0.05)。结论子宫内膜异位症相关卵巢癌患者发病年龄早,临床分期早,血CA125值对于早期子宫内膜异位症相关卵巢癌敏感性差,不能作为判定有无子宫内膜异位症恶变的决定性因素。子宫内膜异位症相关卵巢癌患者同时患有子宫内膜样癌的风险高。Objective To investigate the chnical characteristics of endometfiosis-associated ovarian cancer. Methods The clinical data of fbrty- five patients with endometfiosis associated ovarian cancer and eighty-eight patients with ovarian papillary serous carcinoma were analyzed retrospec- tively. Results The 55.56% of patients in the research group were younger than fifty years old, which was 30.68% in the controls group, and tile diference was significant （ P 〈 0.05 ）. In addition, the percentage of early stage patieuts in the research group and the controls group was 75.56% and 21.59%, respectively （P 〈 0.05 ）. The percentage of normal serum CA-125 at diagnosis in the research group and the controls group was 35.56% and 6.81%, respectively （P 〈 0.05）. Eight patients in the research group （ 17.78% ） had synchronous endometrial cancer, and nobody in the eon- trois group had synchronous endometrial cancer, the difference was significant （P 〈 0.05 ）. Conclusion Patients with endometriosis associated ovarian cancer had the disease at a younger age. There were more early stage patients with endometriosis associated ovarian cancer. Serum CA- 125 al diagnosis may has a poor sensibility with endometriosis associated ovarian cancer, however, it cannot be the determinative factor to assess the malig- nant transtbrmation of endometriosis. Patients with endometriosis associated ovarian cancer had a higher rate of synchronous endometrial cancer.
