白色白内障并发青光眼不同术式的临床观察

摘　　要：目的：对比分析超声乳化和小切口两种不同手术方式治疗白色白内障合并青光眼的临床效果。方法：选取25例30只眼,房角粘连关闭≤1/2的白色白内障并发青光眼患者,随机分为两组,Ⅰ组采用小切口非超声乳化白内障摘除、后房型人工晶状体植入联合虹膜周切12只眼,Ⅱ组白内障超声乳化吸除、后房型人工晶状体植入联合虹膜周切18只眼;观察术前、术后的视力、眼压及并发症情况。结果：术后6个月眼压,Ⅰ组平均为（10.97±0.944）mm Hg,Ⅱ组为（11.11±3.45）mm Hg,与手术前眼压比较差异均有统计学意义（P〈0.05）,两组之间比较无统计学意义（P〉0.05）;术后6个月视力,2只眼≤0.1,0.1~0.4者13只眼,0.5~0.8者15只眼,与手术前视力比较差异有统计学意义（P〈0.05）;早期小切口术后反应较重,术后6个月并发症无显著差异。结论：超声乳化和小切口两种不同手术方式治疗白色白内障合并青光眼的临床效果无显著性差异,均可改善患者视力,有效控制眼压。Objective： To compare and analyze the therapeutic effects of phaceemulsiilcation and small incision cataract surgery on the treatment of white cataract combined with glaucoma. Methods：25 patients（30 eyes） diagnosed white cataract combined with glaucoma with angle closure less than 1/2, were randomly divided into two groups. The group Ⅰ （12 eyes） received small incision cataract surgery and intraocular lens implantation combined with peripheral iridotomy while the group Ⅱ （18 eyes）received phaceemulsification and intraocular lens implantation combined with peripheral /ridotomy. Then the follow-up went for 6 months and the visual acuity, intraocular pressure （IOP） preoperative and postoperative, and the complications were compared and analyzed.Results： Paired t-tests were used in the analyses. During the 6-month follow-up, the average IOP of both groups （group Ⅰ 10.97±0.944mmHg; group Ⅱ11.11±3.45mmHg） were significantly （P〈0.05） lower than that pre-treated, however, there was no significant （P〉0.05） difference in average IOP between group Ⅰ and group Ⅱ after the treatments. Furthermore, the visual acuity was equal or less than 0.1 in 2 eyes, ranged form 0.1 to 0.4 in 13 eyes and form 0.5 to 0.8 in 15 eyes. The visual acuity postoperative significantly （P〈0.05） increased compared with that preoperative. Concurrently, there was no significant difference in the complications events between group Ⅰ and group Ⅱ after the treatments. Conclusion： There is no significant difference in the therapeutic effects on the treatment of white cataract combined with glaucoma between phacoemulsification and small incision cataract surgery. Both methods are effective and safe surgeries to improve visual acuity and decrease IOP in the patients diagnosed white cataract combined with glaucoma.
