摘　　要：目的 对4ng/mL≤PSA≤10ng/mL的患者进行扩大经直肠B超引导下前列腺穿刺活检,从而评价扩大穿刺对4ng/mL≤PSA≤10ng/mL前列腺癌筛选中的临床应用价值.方法 对4ng/mL≤PSA≤10ng/mL的78例患者随机分成2组,分别采用“10 ＋X”（40例）和“6＋X”（38例）经直肠B超引导下前列腺穿刺活检方案,比较两组的阳性率、并发症发生率等.结果 所有患者均顺利完成穿刺,其中“10 ＋X”检出率为32.05％（25/78）,“6＋X”检出率为19.23％ （15/78）,“10 ＋X”增加的PCa检出率为25.00％ （10/40）.结论 经直肠扩大前列腺穿刺是一种对4ng/mL≤PSA≤10ng/mL筛查前列腺癌阳性率较高的方法.Objectives To evaluate the clinical value of prostate cancer（PCa） when PSA is between 4-10ng/ml of patients with enlarged prostate biopsy screening.Methods Clinical data was analyzed of 78 consecutive patients with the PSA between 4-10ng/ml with suspected PCa.The methods of the Transrectal Ultrasound （TRUS）-guided prostate biopsy were ＂10 ＋ X＂ and ＂6 ＋ X＂.The relationship between the PSA relative parameters,including DRE,f/T-PSA,PSAD as well as the size of the prostate volume and the biopsy core number and the positive PCa rate was calculated and analyzed.Results The methods of the Transrectal Ultrasound （TRUS）-guided prostate biopsy with ＂10 ＋ X＂ （40cases） and ＂6 ＋ X＂ （38 cases）in individual systemic biopsies of all patients were performed.The positive rate of ＂10 ＋ X＂ and ＂6 ＋ X＂ was 32.05% and 19.23%,respectively.The increase of＂ 10 ＋ X＂ in cancer detection was 25.00% significantly （P 〈 0.005）.Conclusions Enlarged transrectal prostate biopsy is an effective method for patients with PSA between 4-10ng/ml,and can increase the positive rate.
