摘  要:目的:分析CT灌注成像显示ⅠA期宫颈癌瘤灶的能力,探讨CT灌注参数的特点及其临床应用价值。资料与方法:对20例经活检诊断为ⅠA期宫颈癌的患者进行CT灌注成像,所有患者均在子宫颈CT灌注扫描后经宫颈锥切术病理证实为ⅠA期,选择其中14例正常宫颈组织作为对照。观察分析所得灌注参数图,并测量血流量(BF)、血容量(BV)、平均通过时间(MTT)及表面通透性(PS)值进行统计学分析。结果:ⅠA期宫颈癌组织与正常宫颈组织的BV、PS值有显著性差异(P<0.05),宫颈癌组织的BV、PS值明显高于正常宫颈组织,BF、MTT值差异无统计学意义(P>0.05)。结论:CT灌注图可直观显示ⅠA期宫颈癌病灶,有助于ⅠA期宫颈癌的定位诊断。CT灌注参数可用于宫颈癌的血供分析。Objective: To assess the ability of perfusion CT in showing the foci of stage I A cervical cancer and to ana- lyze the characteristics of the CT perfusion parameters and the contribution of clinical application. Material and Methods: CT perfusion scans were performed in 20 patients who were clinically diagnosed as cervical cancer of Stage I A by biopsy. Then all patients were confirmed as cervical cancer of stage I A by cervical cone cutting operation and pathology. Fourteen cases of cervical cancer patients with normal cervical tissues were used as control. The maps of perfusion parameters were observed and hemodynamic parameters including blood flow(BF), blood volume(BV), mean transit time(MTI') and permeability surface (PS) were calculated and statistically analyzed. Results: There was statistically significant difference in BV and PS be- tween cervical cancer of stage I A and normal cervical tissue (P〈0.05). The BV and PS of stage I A cervical cancer were higher than those of normal cervical tissue. There was no statistical difference in BF and MTr between stage I A cervical cancer and normal cervical tissue(P〉0.05). Conclusion: CT perfusion maps can intuitively show the foci of stage I A cervical cancer which helps the localization diagnosis. CT perfusion parameters can be used to analyze the blood supply of cervical cancer.
