基于锥形束CT肺癌放射治疗两种体位固定技术摆位误差的研究

摘　　要：目的：应用千伏级锥形束CT（k V-CBCT）比较肺癌患者使用负压成型真空垫和热塑体模两种固定技术的摆位误差分析。方法：选取2013.2至2014.3浙江省肿瘤医院60例肺癌放疗患者,在锥形束CT下观测放疗时摆位误差值：将患者随机分为A、B两组每组30例,A组采用负压成型真空垫固定技术,B组采用热塑体模固定技术,每位患者在治疗前使用瓦里安直线加速器的机载影像系统行k VCBCT扫描和配准,采集CBCT图像5-20次,对比计算两组患者在左右（RL）、前后（AP）、头脚（SI）方向的重复摆位误差。结果：A组负压成型真空垫固定技术与B组热塑体模固定技术的摆位误差分别为x轴（左右）方向（2.28±2.6（-0.6-1.4）/1.48±1.22（-0.7-0.6）,P〈0.05）、y轴（头脚）方向（2.52±2.40（-4.5-11.0）/3.23±2.11（-7.0-14.0）,P〉0.05）、z轴（前后）方向（2.84±2.15（-7.0-7.0）/1.59±1.39（-4.0-5.0）,P〉0.05）。结论：在x轴方向热塑体模优于负压成型真空垫,y轴和z轴方向两者无明显差异,建议常规的IMRT患者使用体罩固定。Objective： To evaluate setup errors using kilovoltage cone beam computed tomograph （kV-CBCT） for lung cancers patients treated using vacuum cushions and mask method. Methods：A retrospective study has been carried out for 60 patients from February 2012 to March 2013 in Zhejiang cancer hospital, 60 lung cancer cases were separated into groups A and B. All patients were immobilized with either vacuum cushions （Group A, n=30） or mask （Group B, n=30）.Before irradiation, a kVCBCT scan was performed and image registration was done on a Varian X linear accelerator via OBI system.The images of CBCT were collected from 5 to 20 fractions during the patients radiotherapy. The difference of setup errors between two im- mobilization methods were analyzed. Results： The system setup errors in right-left（RL）,anterior-posterior（AP） and superiorinferior（SI） were（2.28 ±2.6 （-0.6-1.4）/1.48±1.22 （-0.7-0.6）,P〈0.0）, （2.84±2.15 （-7.0-7.0）/1.59±1.39 （-4.0-5.0） ,P〉0.05）, （2.52±2.40 （-4.5-11.0）/3.23±2.11 （-7.0-14.0）,P〉0.05）respecti±cely. Conclusions： The mask device resulted in the least amount of setup error in the X-plane, there were no significant difference in Y-plane and Z-plane. We suggested to use mask to immobilize patients for radiotherapy.
