摘 要:目的研究儿童近视进展与减少视网膜中央凹区多焦视网膜电图(mf ERG)响应的关联。方法 2011年1月到2013年12月共计58名受试者来自我科,年龄7~10岁,平均(9.6±0.2)岁。所有受试者初次就诊、再次就诊和随访1年时接受眼科检查。眼科检查包括客观和主观折射,轴向长度测量和mf ERG记录。结果儿童近视的增加与轴向长度的增加有关(皮尔森相关,r=0.70,P〈0.001)。1年期间近视进展时在49%和96%的对比不同区域的mf ERG响应的变化。随着近视增加,DC振幅在96%之下,DC和IC振幅在49%之下,所有区域的检查值减少。IC振幅在96%中央环1和2的对比减少,但环3和4的振幅略有增加,IC潜时在两种对比减少。相比之下,DC潜时在环1和2的49%对比增加、环3~5近中心区均降低。DC潜时在96%对比在所有检查区域略微增加,除了环1。时域来看,在49%对比时,随着近视进展,环1的DC和IC振幅减少(皮尔森相关,r=0.50~0.53,P〈0.01)。时间域来看随着近视的进展,大部分部位的DC和IC潜时检查未受影响,除了49%对比时环3的DC潜时(r=0.54,P〈0.01)和96%对比时环5的IC潜时(r=0.49,P=0.49)。结论低于10周岁儿童近视进展中,内视网膜功能的中央区和旁中央区恶化。Objective To study the relationship between myopia progression and foveal centralis muhfocal ERG (mtERG) response in children. Methods Totally 26 patients who presented to our clinic between Janum-y 2011 and De- cember 2013 were enrolled in this study. Average age was 10. 6 ± 1.2, range from 9 to 13 years. Ophthalmlc examinations, including objective and subjective refraction, axial length, and mfERG, were followed for 1 year. Results The progres- sion d myopia related to the increase in axial length (Pearson correlation r =-0.70, P 〈0.001 ). Multifocal ERG results showed that DC amplitude was reduced for 96% contrast, and both DC and IC amplitudes were reduced in all fields for 49% contrast. IC amplitude was reduced in ring 1 and ring 2, but slightly increased in ring 3 and ring 4 for 96% contrast. IC latency was shortened for beth 49% contrast and 96% contrasts whereas DC latency increased in ring 1 and ring 2 and reduced in ring 3 - 5 for 49% contrast. DC latency slightly increased in all areas except ring 1 for 96% contrast. In the time domain, the progression of myopia related to the decrease of DC and IC amplitude in ring 1 for 49% contrast ( Pearson correlation, r = 0.50 ~ 0.50, P 〈 0.01 ). DC and IC latencies in most fields were not affected, except DC latency in ring 3 for 49% contrast ( r =0.54, P 〈 0.01} and IC latency in ring 5 ( r = 0.49, P = 0.49) for 96% contrast. Con- clusion In the children aged lower than 10, myopia progression is associated with decrease of central and paracentral fove- al function.
