广泛性焦虑症静息态功能MRI的局部一致性研究

摘　　要：目的:应用静息态功能磁共振成像(rs-fMRI)探讨广泛性焦虑症(GAD)患者脑静息态局部一致性异常。方法:24例GAD患者及24例年龄、性别及教育程度相匹配的健康志愿者行rs-fMRI,应用基于体素的局部一致性(ReHo)分析法,比较两组中脑rs-fMRI的差异(t检验),并对差异脑区的ReHo值与焦虑自评量表(SAS)和汉密尔顿焦虑量表(HAMA)评分进行相关性分析。结果:与对照组比较,GAD组中ReHo值降低的脑区有右侧丘脑、右侧枕中回、左侧额中回和右侧梭状回,ReHo值增高的脑区有右侧额下回、右侧楔前叶、右侧中央前回、左侧额中回和左侧额上回(P<0.001、簇大小>10个体素)。GAD组中仅右侧丘脑的ReHo值与HAMA评分呈显著负相关(r=-0.637,P=0.001)。结论:rs-fMRI可以反映GAD患者脑功能区的局部一致性改变。Objectire：To investigate regional homogeneity （ReHo）of brain in patients with generalized anxiety disor-der （GAD）using resting-state functional MRI （rs-fMRI）.Methods：24 patients with GAD and 24 age-,gender-,and educa-tion-matched healthy volunteers （control group）were included and all of them underwent rs-fMRI.The rs-fMRI data were analyzed with the regional homogeneity method after data conversion.The raw ReHo values were subjected to t-test of voxel wised comparison to derive the resting-state brain function differences between the two groups.The correlation between Re-Ho values of abnormal regions found in GAD group and the scores obtained according to the Self-Rating Anxiety Scale （SAS）and Hamilton Anxiety Scale （HAMA）was analyzed.Results：Compared with the control group,the GAD group showed significantly decreased ReHo value in the right thalamus,right middle occipital gyrus,left middle frontal gyrus,right fusiform gyrus,and increased ReHo value in the right inferior frontal gyrus,right precuneus,right precentral gyrus,left middle frontal gyrus and left superior frontal gyrus （P〈0.001,cluster size＆gt;10 voxels）.The ReHo value of right thalamus in GAD group had a negative correlation with HAMA score （γ=-0.637,P =0.001）.Conclusion：Resting-state fMRI with ReHo method can reflect the abnormal brain activity in the patients with generalized anxiety disorder.
