摘 要:目的探讨DnaJb6在乳腺病变恶性转化过程中的可能作用,分析其表达水平与乳腺癌各病理参数间的关系。方法收集2010-12-01-2011-06-30山东省立医院诊治的98例乳腺癌患者的组织标本、21例不典型乳腺增生组织和15例单纯乳腺增生组织,采用免疫组化技术检测上述标本中DnaJb6的表达及临床意义。结果乳腺癌组织中DNAJB6蛋白高表达率为36.7%(36/98),明显低于不典型乳腺增生组织的71.4%(15/21),差异有统计学意义,χ2=9.966,P=0.002;也明显低于单纯乳腺增生组织的80.0%(12/15),差异有统计学意义,χ2=8.500,P=0.004。3组间比较差异亦有统计学意义,χ2=15.734,P=0.001。不典型乳腺增生组织中DNAJB6蛋白表达率略低于单纯乳腺增生组,差异无统计学意义,χ2=0.343,P=0.558。在乳腺癌组织中,DnaJb6表达与病理类型(P=0.655)、是否绝经(P=0.892)、ER(P=0.655)、PR(P=0.523)、HER-2(P=0.681)、腋窝淋巴结状况(P=0.359)、分子分型(P=0.757)无关,与肿瘤大小(rs=-0.307,P=0.005)、组织学分级呈负相关(rs=-0.364,P=0.007)。结论 DnaJb6的表达下调对乳腺癌的发生可能起促进作用,伴有DnaJb6低表达的乳腺癌恶性程度更高。OBJECTIVE To asssess the function of DnaJb6 in breast cancer initiation and development, analyze the relationship of its expression and clinical data in breast cancer. METHODS A cohort of 98 patients who had primary breast carcinomas treated by surgery at Shandong provincial hospital between December, 2010 and June, 2011 were investigated. Fifteen hyperplasia tissues of usual type and 21 atypical hyperplasia tissues in the same period were collected. The expression of DnaJb6 was evaluated by immunohistochemistry in these samples. RESULTS High expression of DnaJb6 in hyperplasia of usual type, atypical ductal hyperplasia and breast carcinoma was 80. 0% (12/15), 71. 4% (15/21) and 36.7 % (36/98) respectively. Compared with normal hyperplasia and atypical hyperplasia, DnaJb6 was sig nificantly down-regulated in breast cancer(P〈0.05). In atypical hyperplasia, DnaJb6 expressed lower than in normal hyperplasia tissues but didn't reach the significant point(Х^2=0.343 ,P=0. 558). In breast cancer tissues, the expression of DnaJb6 was negatively related with tumor size (rs = -0. 307, P = 0. 005) and histological stage (rs = -0. 364, P = 0. 007), but had no association with menstrucal status(P=0. 892), pathologic type(P=0. 655), ER expression(P= 0. 655), PR status(P=0. 523), HER-2 status(P=0. 681), axillary node status(P=0. 359) and molecular subtypes(P= 0. 757). CONCLUSIONS Down-regulation of DnaJb6 may contribute to the initiation of breast cancer. Breast cancer with low expression of DnaJb6 tends to be more aggressive. DnaJb6 may be an indicator to determine the prognosis of breast cancer.
