30例原发性胃淋巴瘤的诊断及外科治疗

摘　　要：目的探讨原发性胃淋巴瘤(PGL)的诊断和外科治疗策略。方法回顾性分析经手术治疗的30例PGL患者的临床资料。结果肿瘤位于胃底部5例,胃体部9例,胃窦部12例,多部位4例。病变呈浸润型6例,溃疡型11例,结节型4例,肿块型9例。临床分期为Ⅰ期11例,Ⅱ期10例,Ⅲ期5例,Ⅳ期4例。行根治性切除术23例,短路手术4例,减瘤手术3例。25例获得随访,随访时间3年以上,1年和3年生存率分别为96.0%和84.0%。结论 PGL临床表现不典型,具备手术指征且不能明确诊断的患者应行手术治疗。Objective To analyze the diagnosis and surgical treatment of 30 cases with primary gastric lymphoma（PGL）.Methods Clinical data of 30 PGL patients underwent surgical treatment were retrospectively analyzed.Results Tumors located at stomach bottom in 5cases,gastric body in 9cases,gastric antrum in 12 cases,and more parts of stomach in 4cases.The lesion type was infiltrating in 6cases,ulcerative in 11 cases,nodule in 4cases and lump in 9cases.The patients in clinical stages ofⅠ,Ⅱ,Ⅲ and Ⅳ were 11 cases,10cases,5cases and 4cases,respectively.The surgery performed was radical resection in 23 cases,bypass resection in 4cases,and palliative resection in 3cases.Twenty-five cases with PGL were followed up for more than 3years,which showed 1-and 3-year survival rates of 96.0% and 84.0%.Conclusion Due to atypical clinical manifestations of PGL,surgical treatment should be considered in the patients with surgical indication,but no clear diagnosis.
