4种根管冲洗液清除根管玷污层的研究

摘　　要：目的评价4种根管冲洗液清除根管玷污层的效果。方法 20个无龋单根前牙随机分为4组,A组:60℃、3%次氯酸钠(Na Cl O)和MTAD冲洗;B组:60℃、3%Na Cl O和Smear Clear冲洗;C组:60℃、3%Na Cl O和17%乙二胺四乙酸(ethylene diamine tetraacetic acid,EDTA)冲洗;D组:3%双氧水(H2O2)和生理盐水(0.9%Na Cl)冲洗。扫描电镜下观察根尖1/3、根中1/3和根上1/3玷污层清除情况,按照根管玷污层清除分级计分标准进行统计学分析。结果 A、B、C、D组根上1/3区玷污层分值为1.18±0.21、1.10±0.15、1.20±0.18、3.52±0.11;根中1/3区玷污层分值为1.63±0.48、1.83±0.24、1.73±0.15、4.01±0.29;根尖1/3区玷污层分值为1.71±0.45、1.74±0.14、3.33±0.24、4.25±0.32。在根上1/3和根中1/3区A、B、C各组间差异无统计学意义(P>0.05),3组与D组差异有统计学意义(P<0.05);根尖1/3区A、B组间差异无统计学意义(P>0.05),A、B组与C、D组差异有统计学意义(P<0.05)。结论 60℃、3%Na Cl O与MTAD或Smear Clear联合使用均能有效去除根管玷污层。Objective To compare the effects of four different irrigating methods on removal of smear layer in root canal walls. Methods Twenty single-rooted human anterior teeth were randomly divided into four groups. Group A： irrigation with MTAD and 3% NaCIO （sodium hypochlorite） at 60 ℃ ; Group B： irrigation with SmearClear and 3% NaC10 at 60 ℃; Group C： irrigation with 17% EDTA amt 3% NaClO at 60 ℃; Group D： irrigation with 3% H2O2 and 0, 9% NaCI. After final irrigation, all tbe samples were prepared for SEM （scanning electron microscopy） analysis to evaluate the cleaning effect of the coronal, middle and apical thirds of radicular dentin by determining the presenee of smear layer. Results At the coronal and middle thirds, there was no significant difference between gruup A, B and C. All of them were equally efficient in the renloval of smear layer except D which still had smear laver. There was significant difference between group A, B, C and group D. （ At the coronal third, the score of smear layer for group A, B, C and D were 1.18 ±0.21 , 1.10±0.15, 1.20 ±0.18 and 3.52 ±0. 11 separately; at the middle third, the score of smear layer for group A, B, C and D were 1.63±0.48, 1.83 ±0.24, 1.73 ±0.15 and 4.01 ±0.29 separately） , at the apical third, group A and B showed better smear layer removing ability than group C and D. There was significant difference between group A, B and group C, D. （ At the apical third, the score of smear layer for group A, B, C and D were 1.71 ±0.45, 1. 74 ± 0. 14, 3.33 ±0.24 and 4.25 ±0.32 separately）. Conclusion Irrigation with 60℃, 3% NaCIO comhined with MTAD or Smearelear is more efficient than other methods in the removal of smear layer from the root canal system.
