摘 要:目的探讨血清促红细胞生成素(EPO)、铁蛋白、叶酸和维生素B12在急性白血病(AL)患者中的水平变化及临床意义。方法 102例急性白血病患者根据临床资料分成:初治组42例、缓解组34例、复发组26例。另选取30名健康体检者为正常对照组。应用化学发光法检测血清EPO、铁蛋白、叶酸和维生素B12水平。结果1急性白血病中血清EPO、铁蛋白及维生素B12水平较正常组显著升高(P<0.01)。2血清EPO、铁蛋白及维生素B12三项联合检测在急性白血病中的阳性率(90.2%)明显高于各指标单项阳性率(P<0.05)。3血清EPO、铁蛋白及维生素B12水平在急性白血病初治时升高,缓解后降低,复发时复升;而叶酸水平在初治时降低,缓解时升高,差异均具有统计学意义(P<0.05)。4血清EPO与铁蛋白水平之间、铁蛋白与及维生素B12水平之间呈正相关(r=0.318,0.389;P<0.05)。结论血清EPO、铁蛋白、叶酸和维生素B12水平的检测对急性白血病的诊断及病情监测有一定参考价值。[ Objective ] To explore thelevels variation of serum erythropoietin (EPO), ferritin, folate and vitamin B12 and its clinical significance in patients with acute leukemia. [Methods ] According to the clinical data, 102 cas- es of patients with acute leukemia (AL) were divided into three groups: 42 cases of initial treatment group, 34 cases of remission group and 26 cases of recurrent group. Another 30 healthy subjects were selected as the control group. The levels of serum EPO, ferritin, folate, vitamin B12 levels were measured by Chemiluminescence. [ Results ] The levels of serum EPO, ferritin, vitamin B12 in acute leukemia patients were significantly higher than normal control group (P 〈0.01). The positive rate of combined detection of serum EPO, ferritin and vitamin B12 reached 90.2%, which was significantly higher than that of the single test(P 〈0.05). The serum EPO, ferritin and vitamin B~2 levels in acute leukemia patients would raise at initial treatment period, then reduce after alleviate, but raise again once re- current; whereas the folate level would decreased at the beginning of the disease, and then elevated in remission pe- riod, the differences were statistically significant(P 〈0.05). There were some positive correlation between the levels of serum EPO and ferritin, Folate and vitamin B12(r =0.318,0.389; P 〈0.05). [ Conclusions] The detection of serum EPO, ferritin, folate, vitamin B12 levels is helpful to the diagnosis of acute leukemia and also has a certain value formonitoring the condition changes in acute leukemia.
