摘  要:[目的]探讨改善酒精性胰腺腺泡细胞分泌功能不足的分子靶点。[方法]1将48只大鼠随机分为4组,每组12只。饮酒6个月组(A1组),饮酒6个月对照组(A2组),戒酒3个月组(B1)组,戒酒3个月对照组(B2组);2苏木精-伊红染色和电镜观察戒酒后酒精性胰腺腺泡细胞的形态变化;3采用酶法测定胰腺组织的淀粉酶、脂肪酶含量;4免疫荧光法和免疫印迹检测CCK-A受体在胰腺的表达;5放射免疫分析法(RIA)测定胰腺和小肠组织中的胆囊收缩素(CCK)含量;6用比色法测定血浆和小肠食糜中游离脂肪酸及食糜中三酰甘油含量。[结果]与A2组大鼠相比,A1组电镜下见腺泡细胞超微结构出现了明显的变化,如线粒体水肿,髓样小体形成,粗面内质网扩张,酶原颗粒减少,易见脂滴;A1组大鼠饮酒6个月后胰腺淀粉酶、脂肪酶水平、小肠和胰腺的CCK水平均较A2组显著下降。B1组胰腺组织学及腺泡细胞超微结构的病变较B2组并无明显改善,其胰腺淀粉酶、脂肪酶水平、小肠和胰腺的CCK水平仍低于B2组。[结论]慢性酒精对胰腺腺泡细胞结构及功能的损伤在戒酒后仍不能自然恢复;补充消化酶,可改善其外分泌功能。[Objective]The molecular target for improving pancreatic exocrine function in pancreatic in- sufficiency rats induced by long-term alcohol administration was investigated. [Methods]①Forty-eight rats were randomly divided into 4 groups with 12 rats in every group. In group A1, alcohol was given orally for 6 months. In group A2, water was given for 6 months. In the other two groups(B1, B2), the protocol were the same as group A1 except alcohol or water were given for 6 months and 3 more months period of investigation was contineous. ②The tissue sections of pancreas were stained with Heamotoxin&Eosine and studied under a light microscope in order to observe the histological changes of pancreas. And the ul- trastructrural changes of pancreatic acinar cells was detected by an electron microscope. ③Pancreatic amyl-ase and lipase activities were measured by enzymic method. ④Immunofluorescence and Western-blot were used to detect CCK-A receptor in rat pancreas. ⑤Cholecystokinin (CCK) was quantified by radioimmuno- assay. 6. Colorimetric method was adopted to quantify triglyceride (TG) in chymus, free fatty acids in plasma and chymus. [Results]①The histological changes of the pancreas and ultrastructrural changes in the pancreatic acinar cells were not improved even after alcohol withdrawal. In contrast to concurrent control, which amylase and lipase was (87.47±22.18) and(8.01±2.96 )respectively, the amylase and lipase levels in the pancreas notably decreased in alcohol withdrawal group which amylase and lipase was (62.03± 32. 04),(5.43±2.81),P〈0. 05. ②After alcohol withdrawal, CCK in both small intestine ([157.40±47.93]pg/mg protein) and pancreas ([20.33±16.19] pg/mg protein) did not restore to the normal, which was (209.33±41.03)pg/mg protein in the small intestine and 69.31±24.54 pg/mg protein in the pancre- as in concurrent control(P〈0.01).
