2型糖尿病合并急性脑梗死患者血清内脂素的测定及阿托伐他汀的干预作用

摘　　要：目的 通过测定2型糖尿病（T2DM）合并急性脑梗死（ACI）患者血清内脂素含量及生化、糖化血红蛋白指标变化,探讨其临床意义及阿托伐他汀的干预作用.方法 单纯T2DM组50例和T2DM合并ACI组患者50例均给予立普妥进行治疗;另选择50健康人群作为对照组给予安慰剂,测定给药前后血清内脂素含量及生化、糖化血红蛋白指标.结果 单纯T2DM组与T2DM合并ACI组血清内脂素、糖化血红蛋、空腹胰岛素、总胆固醇、高密度脂蛋白胆固醇（HDL-C）、空腹血糖（FBG）、三酰甘油（TG）、胰岛素抵抗指数（Homa-IR）均明显高于健康人群（P均＜0.05）,其中T2DM合并ACI组血清内脂素、TG、FBG、Homa-IR明显高于单纯T2DM组,HDL-C低于单纯T2DM组（P均＜0.05）;Logistic逐步回归分析结果表明,血清内脂素和Homa-IR是合并ACI的危险因素.经立普妥治疗6个月后T2DM组与T2DM合并ACI组Homa-IR与内脂素水平均较干预前有所降低（P＜0.01）.结论 血清内脂素是T2DM合并ACI的高危因素,他汀类药物阿托伐他汀可降低血清中内脂素含量,进而降低Homa-IR的含量,延缓疾病发展.Objective To detect serum visfatin and biochemical,glycosylated hemoglobin index changes in patients with type 2 diabetic combined acute cerebral infarction,and investigate its clinical effect and the intervention effect of atorvastatin.Methods Fifty cases of simple T2DM group and 50 cases of T2DM combined with ACI patients were treated with atorvastatin.Another 50 healthy people as control group were given placebo.Serum visfatin content and biochemical,glycosylated hemoglobin indicators were measured before and after administration.Results Visfatin HDL-C,HbA1c,FINS,FBG,TG,Homa-IR values of T2DM group or T2DM with ACI group were significantly higher than the healthy population （P all ＜ 0.05）.Serum visfatin,TG,FBG,HOMA-IR value of T2DM with ACI group was significantly higher than that in T2DM group,HDL-C was lower than that in T2DM group （P all ＜ 0.05）.Logistic regression analysis showed that serum visfatin and Homa-IR was a merger of ACI risk factors.Homa-IR and visfatin levels decreased significantly by the intervention of atorvastatin in the T2DM group and T2DM combined with ACI groups （P ＜ 0.01）.Conclusions Serum visfatin is the risk factors of T2DM combined with ACI,atorvastatin can reduce serum visfatin content,thereby reduce the content of the HOMA-IR,delay the development of the disease.
