956例老年人脂肪肝患者的中医体质分型及相关因素临床分析

摘　　要：目的:探讨社区老年人脂肪肝中医体质的分布特点及与相关指标的关系,以便更有利于指导患者进行中医药防治。方法:对机场社区的956例60岁以上人群的脂肪肝患者进行中医体质判定及相关体检指标(BMI、ALT、AST、TG、TC)进行统计。结果:老年人脂肪肝患者中医体质分型中痰湿质321例,占33.58%;气虚质189例,占19.77%;湿热质155例,占16.21%。未见特禀质。956例老年人脂肪肝患者中超重及肥胖的人数达到840例,占87.87%,且各体质类型的体质指数总体分布不同,经秩和检验P<0.001,差异有统计学意义。湿热质ALT、AST最高,与其他各型比较均有统计学意义(P<0.05);痰湿质TG最高,与其他各型比较均有统计学意义(P<0.05)。结论:痰湿质和气虚质是老年人脂肪肝的易感体质。痰湿质的BMI、TG水平最高,符合"痰""湿"致病的脂肪肝中医病机解释,湿热质的ALT、AST水平最高,其次为痰湿质,提示这两种体质的老年人脂肪肝患者的肝脏损害程度相对较严重。Objective： In order to guide the patients to prevent and cure elders’ fatty liver by traditional Chinese medicine,they analyzed and discussed the relationship between distribution characteristics of elder’s physique and relevant data in community. Methods： Making statistics on 956 elders above 60 years old with fatty liver in airport community（ BMI,ALT,AST,TG,TC）. Results： Elders’ traditional Chinese physique with fatty liver can be divided into several types. Phlegm- dampness constitution： 321 cases,taking 33. 58%. Qi- asthenia constitution： 189 cases,taking 19. 77%. Dampness- heat constitution： 155 cases,taking 16. 21%. No ＂Tebing constitution＂ appeared. Within 956 patients,840 cases were overweight or fat,taking87. 87%,different physique data distribution,after inspection of P 〈 0. 001,it was meaningful to make statistics,compared with other constitution,Dampness- heat constitution had highest ALT and AST（ P 〈 0. 05）,but phlegm- dampness had highest TG,also meaningful compared with other constitutions. Conclusion： Phlegm- dampness constitution and Qi- asthenia constitution are allergic groups. Phlegm- dampness constitution has highest BMI and TG,commits to the traditional Chinese explanation for Phlegm and Qi causing fatty liver. Dampness- heat constitution has highest ALT and AST,then phlegm- dampness constitution,the result shows elder patients with both constitutions are comparatively serious damaged on liver.
