经根管使用派丽奥治疗牙周牙髓联合病变的X线观察

摘　　要：目的观察经根管使用盐酸米诺环素(派丽奥)治疗牙周牙髓联合病变的术后不同时间X线变化,以评价派丽奥对牙周牙髓联合病变的疗效。方法牙周牙髓联合病变患牙52颗,所有患牙术前摄X线根尖片,经基础牙周治疗,开髓揭顶,Protaper系统逐步深入法根管预备,按根管预备时间随机交替分为实验组和对照组。实验组采用派丽奥经根管-根尖-牙周途径给药,对照组采用常规氢氧化钙根管封药,同时牙周袋内注入派丽奥对患牙进行治疗。两组均采用冷牙胶侧压充填,摄片观察术后0 d、3个月、6个月的根尖片(平行投照)显示的根尖暗影的最大水平直径(X)和垂直方向的直径(Y)以及牙槽骨吸收的最大垂直高度(H)和最大水平宽度(L)的变化。结果术后0 d,两组的X、Y、H、L无显著差异(P>0.05);术后3、6个月,实验组的X、Y、H、L缩小范围较同时间对照组的更大(P<0.05),且时间越久,缩小差异越大。结论经根管使用盐酸米诺环素治疗牙周牙髓联合病变是一种有效可行的方法,且较传统氢氧化钙封药根管表现出更好的远期效果,值得临床推广。Objective To observe the postoperative changes on X-ray image of periodontal endodontic lesions treated with minocycline hydrochloride（ Periocline） through root canal and to evaluate the curative effect of Periocline on periodontal endodontic lesions. Methods Dental periapical X-ray images of for 52 teeth with periodontal endodontic lesions were taken before the operation.After the basic periodontal treatment,the dental pulp was exposed with the top of tooth uncovered and the Protaper system was used to gradually get into the depth to prepare the root canal. The teeth samples were divided into experimental group and control group randomly and alternately according to the preparation time of the root canal. Periocline was administered to the patients in the experimental group through the route of root canal,root tip,and periodontium,while conventional calcium hydroxide intracanal medication was provided for the patients in the control group with Periocline being injected into the periodontal pocket for treatment.Cold gutta-percha lateral condensation was adopted for both groups,and then periapical X-ray images（ parallel projection） were taken on the current day,the 90 th day,and the 180 th day after the operation in order to observe the changes of the maximum horizontal diameter（ X） and vertical diameter（ Y） of the root tip shadow shown in the images,and changes of the maximum vertical height（ H） and the maximum horizontal width（ L） of alveolar bone absorption. Results On the current day after the operation,there were no significant differences in X,Y,H,and L between the two groups（ P〉 0. 05）; on the 90 th day and 180 th day after the operation,the reduced ranges of X,Y,H,L in the experimental group were greater than those in the control group at the same time（ P〈 0. 05）,and the longer the time was,the greater the differences were.
