摘  要:目的比较载脂蛋白A-1(ApoA-1)在前列腺癌、前列腺增生组织中的表达情况,探索前列腺疾病鉴别诊断的新型标记物。方法应用免疫组织化学方法检测前列腺癌(13例)和前列腺增生(37例)组织中ApoA-1的表达情况,分析其与血清前列腺抗原系列总前列腺特异性抗原(tPSA)、游离前列腺特异性抗原(fPSA)、fPSA与tPSA比值(fPSA/tPSA)及血脂总胆固醇(CHOL)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)等参数的相关性,研究ApoA-1在前列腺特异抗原(PSA)灰区(4~10ng/mL)对前列腺癌诊断的特异度及血清胆固醇水平升高在前列腺癌发生发展过程中的作用。结果 ApoA-1在前列腺癌患者组织中的表达水平明显高于前列腺增生患者,比较差异有统计学意义(t=-6.182,P〈0.05)。在PSA不同水平的各个区间内,ApoA-1在前列腺癌组织中的表达明显高于相同PSA水平的前列腺增生组织,差异有统计学意义(P〈0.05)。前列腺癌患者血清LDL-C水平为(3.02±0.79)mmol/L,明显高于前列腺增生患者的(2.14±0.78)mmol/L,差异有统计学意义(P〈0.05);血清HDL-C水平为(1.27±0.33)mmol/L,明显低于前列腺增生患者的(1.70±1.02)mmol/L,差异有统计学意义(P〈0.05),ApoA-1的水平与胆固醇水平之间具有相关性(r=0.332,P=0.036)。结论 ApoA-1在前列腺癌和前列腺增生中表达具有明显差异,可作为二者的鉴别诊断标记物。Objective To investigate the different expression of apolipoprotein A-I(ApoA-1) in prostate cancer and benign pros- tatic hyperplasia,and to explore the new indicators of prostate diseases. Methods The immunochemistry was used to detect the expression of ApoA-1 in prostate cancer tissues (13 cases)and benign prostatic hyperplasia tissues(37 cases). The correlations between the level of ApoA-1 and the serum prostate antigen series total prostate specific antigen(tPSA), free prostate specific antigen (fPSA), the ratio of fPSA and total PSA(fPSA/tPSA), cholesterol(CHOL), triglyeeride(TRIG), low-density lipoprotein-cholester- ol(LDL-C) and high-density lipoprotein-cholesterol(HDL-C) levels were analyzed. Results In 13 cases of prostate cancer,ApoA-1 showed strongly positive expression, while ApoA-1 was weak positive stained or even negative in 37 cases of benign prostatic hyper- plasia,a significant difference was observed between them(t= -6. 182, P〈0.05). Within the same range of PSA levels, ApoA-1 was obviously higher in the cancer group than that of the benign prostatic hyperplasia patients (P〈0.05). The LDL-C level of prostate cancer group (3.02 ± 0.79)mmol/L was higher than that of benign prostatic hyperpalsia group(2.14 ±0.78)mmol/L, the difference was statistically significant(P〈0.05). The HDL-C level of prostate cancer group (1. 274-0.33)mmol/L was lower than that of benign prostatic hyperpalsia (1.70±1.02)mmol/L,The difference was statistically significant (P〈0.05). There was a sig- nificant correlation between the level of ApoA-1 and the level of cholesterol(r=0. 332,P〈0.05). Conclusion The expression of ApoA-1 is obvious different between the patients with prostate cancer and the patients with benign prostatic hyperplasia.
