DC-CIK联合化疗治疗进展期胃癌45例的临床疗效评价

摘　　要：目的研究树突状细胞(dendritic cell,DC)共培养细胞因子诱导的杀伤细胞(cytokine-induced killer cell,CIK)联合化疗用于进展期胃癌术后治疗的临床疗效。方法选取90例进展期胃癌患者进行手术治疗,术后随机分为观察组(45例,DC-CIK生物治疗联合化疗)和对照组(45例,常规化疗)。观察两组患者治疗前后T细胞亚群、细胞因子的变化、临床疗效和不良反应等。结果观察组患者经过DC-CIK联合化疗方案治疗后,CD3+CD4+T淋巴细胞百分比、CD4+/CD8+比率、NK细胞百分比和细胞因子IFN-γ均较治疗前、对照组治疗后显著升高(P<0.05)。观察组1、2、3年累积复发率显著低于对照组(P<0.05);观察组1年累积生存率明显高于对照组(P<0.05)。DC-CIK回输过程中少量患者出现轻度发热,未见明显不良反应。结论进展期胃癌患者术后采用DC-CIK联合化疗治疗方法能够提高免疫细胞功能、减少肿瘤复发率,不良反应少,具有较强的临床推广价值。Objective To evaluate the immune mechanism and clinical effect of immunotherapy of dendritic cell （DC） and cytokine-induced killer cell （CIK） combined with chemotherapy on advanced gastric cancer. Methods Ninety patients with advanced gastric cancer were randomly divided into two groups after operation. Chemotherapy group （45 cases） were treated by chemotherapy only, 45 patients in combined group were treated by adoptive immunotherapy （DC-CIK） combined with chemotherapy. The immune function, therapeutic effect and side effects were compared be- tween two groups. Results The percentages of CD3＋ CD4 ＋ T lymphocytes, the CD4 ＋/CD8 ＋ ratio, NK cells and the level of IFN-γ in the combined group were identified to be significantly increased after treatment compared with before treatment and control group （P 〈 0.05）. The cumulative recurrence rates over the three years in the observation group were significantly lower than those in the control group （P 〈 0.05 ）. The cumulative survival rate over one year in the observation group was significantly higher than that in the control group （P 〈 0.05 ）. In the process of autologous DC- CIK treatment, some cases appeared the symtoms of slight fever, but had no evident adverse reactions. Conclusion The technique of DC-CIK combined with chemotherapy significantly improved the immune cell function, reduced postop- erative tumor recurrence with high security attributes, and is worthy to be widely spread.
