内镜粘膜切除术(EMR)治疗结肠无蒂息肉的疗效观察

摘　　要：目的 探讨内镜粘膜切除术治疗结肠无蒂息肉的效果及安全性。方法回顾分析该院2011年5月—2012年5月收治的160例180枚结肠息肉患者,分别行EMR和单纯电切术,观察术中、术后并发症及术后复发情况,分为EMR组和电切组,每组80例。结果 EMR组未能完整切除、术中出血、切口过深共13例,无术后出血、穿孔、息肉复发病例,均送病理检查,并发症总发生率16.25%。电切组未能完整切除、术中出血、术后出血、穿孔、息肉复发、无法送病理检查者共40例,并发症总发生率50.00%。EMR组总并发症明显低于电切组(P<0.05)。结论 EMR术治疗有效、安全,操作简单,具有诊断及治疗双重意义,是一种安全有效的治疗方法。Objective To observe the effect and safety of endoscopic mucosal resection in the treatment of sessile colon polyps.Methods A retrospective analysis was conducted on 160 cases with colonic polyps（180 colonic polyps） admitted in our hospital.The patients were divided into two groups, the EMR group and the electrotomy group with 80 cases in each, treated by EMR, electrotomy, respectively. And the intraoperative state, postoperative complications and postoperative recurrence of the two groups were observed. Results In the EMR group, a total of 13 patients had incomplete excision, or intraoperative bleeding or too deep incision, no postoperative bleeding, perforation, polyp recurrence cases, all the excisional polyps were examined pathologically, the overall incidence of complications was 16.25%. In the electrotomy group, a total of 40 cases had incomplete excision, intraoperative bleeding, postoperative bleeding, perforation, polyp recurrence or the excisional polyps could not send for pathology examination, the overall incidence of complications was 50.00%. The EMR group had much less complications than the electrotomy group（P0.05）. Conclusion EMR is safe, effective, easy to operate with the value of diagnosis and treatment. It is a safe and effective treatment method.
